目的:探讨糖化血红蛋白(HbA1c)的测定对妊娠期糖尿病(GDM)诊断的价值。方法:通过检索中国期刊全文数据库、文献追溯以及手工检索的方式,全面检索1979~2010年国内有关糖化血红蛋白对妊娠期糖尿病诊断价值的相关文献资料,应用RevMan 5.0软件,对筛选出的符合要求的17篇文献进行Meta分析。结果:所选资料间异质性较显著,故采用随机效应模型,分析得出OR合并值为61.50,95%CI为(30.77,122.91),总体效应检验有统计学意义(Z=11.66,P<0.00001)。失安全系数Nfs值为8 220.045。结论:从该次分析结果可以看出,糖化血红蛋白测定可以普及应用于临床妊娠期糖尿病的筛查和诊断。
Objective: To investigate the value of determination of glycosylated hemoglobin (HbA1c) in the diagnosis of gestational diabetes mellitus (GDM). Methods: The related literatures about the diagnostic value of glycated hemoglobin in gestational diabetes mellitus in China from 1979 to 2010 were retrieved through the retrieval of full-text databases of Chinese journals, literature review and hand searching methods. Using RevMan 5.0 software, 17 articles for Meta analysis. Results: The heterogeneity of the selected data was significant. Therefore, using the random effect model, we found that the OR value of OR was 61.50 and the 95% CI was (30.77, 122.91). The overall effect test was statistically significant (Z = 11.66, P <0.00001). The loss of safety factor Nfs value is 8 220.045. Conclusion: From the results of this analysis, it can be seen that HbA1c can be widely used in the screening and diagnosis of clinical gestational diabetes mellitus.
