目的探讨红细胞分布宽度(RDW)、颈动脉内膜中层厚度(CIMT)及血清脂蛋白(a)〔Lp(a)〕水平与急性脑梗死(ACI)患者病情严重程度的关系。方法选取2013年1月—2015年1月贵港市中西医结合骨科医院收治的ACI患者120例为病例组,另选取同时期在本院体检健康成年人45例为对照组。根据脑梗死面积将病例组患者分为大面积亚组24例、中面积亚组42例及小面积亚组54例;根据美国国立卫生研究院卒中量表(NIHSS)评分将病例组患者分为轻型梗死亚组57例、中型梗死亚组44例及重型梗死亚组19例。检测各组患者RDW、CIMT及血清Lp(a)水平,并分析其与NIHSS评分的相关性。结果病例组患者RDW、CIMT及血清Lp(a)水平高于对照组(P<0.05);大面积亚组患者RDW、CIMT及血清Lp(a)水平高于中面积亚组及小面积亚组,中面积亚组患者RDW、CIMT及血清Lp(a)水平高于小面积亚组(P<0.05);重型梗死亚组患者RDW、CIMT及血清Lp(a)水平高于中型梗死亚组,中型梗死亚组患者RDW、CIMT及血清Lp(a)水平高于轻型梗死亚组(P<0.05)。Pearson相关性分析显示,RDW、CIMT及血清Lp(a)水平均与NIHSS评分呈正相关(P<0.05)。结论 RDW、CIMT及血清Lp(a)水平可以作为反映ACI患者病情严重程度的指标。
Objective To investigate the relationship between RDW, CIMT and serum lipoprotein (a) [Lp (a)] and the severity of acute cerebral infarction (ACI). Methods From January 2013 to January 2015, 120 patients with ACI in Guigang Hospital of Integrated Chinese and Western Medicine were selected as the case group, and 45 healthy adults in our hospital were selected as the control group during the same period. According to the area of cerebral infarction, the patients in the case group were divided into a large area group (24 cases), a medium area group (42 cases) and a small area group (54 cases). According to the National Institutes of Health Stroke Scale (NIHSS) score, 57 cases of light infarction subgroup, 44 cases of medium infarction subgroup and 19 cases of severe infarction subgroup. The levels of RDW, CIMT and serum Lp (a) in each group were detected and their correlations with NIHSS scores were analyzed. Results The levels of RDW, CIMT and serum Lp (a) in case group were significantly higher than those in control group (P <0.05). The levels of RDW, CIMT and serum Lp (a) in large subgroup were higher than those in middle and small subgroup (P <0.05). The levels of RDW, CIMT and serum Lp (a) in the medium subgroup were higher than those in the small subgroup (P <0.05) The levels of RDW, CIMT and serum Lp (a) in patients with mid-infarct subgroup were higher than those in mild infarct subgroup (P <0.05). Pearson correlation analysis showed that RDW, CIMT and serum Lp (a) levels were positively correlated with NIHSS scores (P <0.05). Conclusions The levels of RDW, CIMT and serum Lp (a) can be used as indicators of the severity of ACI.
