目的观察急性心肌梗死病人的血浆脑钠肽前体水平和同型半胱氨酸水平的变化,探讨其与冠状动脉病变程度的关系。方法回顾性分析2013年1月—2015年10月我院收治的急性心肌梗死病人269例,按照冠状动脉造影结果分为3组:单支病变组、双支病变组和三支病变组,检测并比较各组的脑钠肽前体、同型半胱氨酸、超敏肌钙蛋白Ⅰ、左室射血分数及Gensini积分。结果双支病变组的脑钠肽前体、同型半胱氨酸、超敏肌钙蛋白Ⅰ及Gensini积分均高于单支病变组,三支病变组的脑钠肽前体、同型半胱氨酸、超敏肌钙蛋白Ⅰ及Gensini积分均高于双支病变组和单支病变组,有统计学意义(P<0.05);左室射血分数则为双支病变组低于单支病变组,三支病变组低于双支病变组和单支病变组(P<0.05)。结论急性心肌梗死病人血浆脑钠肽前体、同型半胱氨酸水平与冠状动脉病变程度有关,其水平越高,冠状动脉病变程度越重,可通过其水平来判断冠状动脉病变程度。
Objective To observe the changes of plasma brain natriuretic peptide and homocysteine levels in patients with acute myocardial infarction (AMI) and to explore the relationship between plasma BNP level and the degree of coronary artery lesion. Methods A retrospective analysis of 269 cases of acute myocardial infarction admitted to our hospital from January 2013 to October 2015 was divided into three groups according to coronary angiography results: single vessel disease group, double vessel disease group and three vessel disease group The brain natriuretic peptide precursor, homocysteine, troponin I, left ventricular ejection fraction and Gensini scores were compared between the groups. Results The levels of brain natriuretic peptide, homocysteine, high-sensitivity troponin I and Gensini scores in the double-vessel lesion group were significantly higher than those in the single vessel lesion group. The BNP precursor, homocysteine Acid, high-sensitivity troponin I and Gensini scores were higher than the double-vessel disease group and single-vessel disease group, with statistical significance (P <0.05); left ventricular ejection fraction was double-vessel lesion group than single vessel disease Group, three lesions were lower than the double-vessel disease group and single vessel disease group (P <0.05). Conclusion Plasma BNP precursor and homocysteine levels in patients with acute myocardial infarction are related to the severity of coronary artery disease. The higher the level of coronary artery disease, the more severity of coronary artery disease. The severity of coronary artery disease can be judged by its level.
