目的:探讨彩色多普勒成像(colorDopplerimaging,CDI)技术对脉络膜肿瘤的诊断价值。方法:用高分辨率CDI对49例脉络膜肿瘤内血流特征作定性、定量分析。结果:25例脉络膜黑色素瘤中24例(96%)和5例脉络膜转移癌中3例(60%)显示肿瘤内含枝状分布的血流,频谱呈较高阻力的波形,8例脉络膜血管瘤显示肿瘤内呈“血管池”样充满或弥散星点分布的丰富血流,频谱含呈较低阻力动脉血流和静脉血流波形;脉络膜黑色素瘤和脉络膜血管瘤在肿瘤血管舒张期血流速度和阻力指数上差异有显著性(P<0.05);肿瘤大小与肿瘤血管的血流速度无明显相关关系(P>0.05)。而7例脉络膜积血、4例局限性视网膜下出血未见病变内血流。结论:用CDI分析脉络膜肿瘤血流特征有助于其鉴别诊断。
Objective: To investigate the diagnostic value of color Doppler imaging (CDI) in choroidal neoplasms. Methods: The characteristics of blood flow in 49 cases of choroidal neoplasm were qualitatively and quantitatively analyzed by high resolution CDI. Results: Twenty-five patients (96%) in choroidal melanoma and three patients (60%) in five choroidal metastatic carcinomas showed a dendritic distribution of blood flow in the tumor with a waveform of higher resistance and eight cases of choroidal blood vessels Tumor showed a “blood pool” -like tumor-filled or diffuse interstitial distribution of abundant blood flow, the spectrum showed lower resistance arterial blood flow and venous flow waveform; choroidal melanoma and choroidal hemangioma in the tumor vasodilatory blood flow The difference of velocity and resistance index was significant (P <0.05). There was no significant correlation between tumor size and blood flow velocity of tumor vessels (P> 0.05). The seven cases of choroidal hemorrhage, 4 cases of subretinal hemorrhage no blood flow within the lesion. Conclusion: The CDI analysis of choroidal tumor blood flow is helpful for its differential diagnosis.
