目的:观察比较炎琥宁和利巴韦林治疗小儿重症上呼吸道感染的疗效及安全性。方法:选择重症上呼吸道感染患儿116例,随机分为观察组和对照组各58例。两组均给予退热、多饮水、易消化的半流食或软食为主等常规处理,对照组给予利巴韦林注射液静脉滴注,观察组给予炎琥宁注射液静脉滴注。比较两组上呼吸道感染症状消失时间,治疗前后血清肿瘤坏死因子-α(TNF-α)、白介素6(IL-6)水平,CD3~+、CD4~+及CD4~+/CD8~+及不良反应发生情况。结果:观察组鼻塞、咳嗽、咽喉红肿消失时间显著或非常显著短于对照组(P<0.05,P<0.01),两组退热时间、腹泻停止时间差异不显著(P>0.05)。治疗后,两组血清TNF-α及IL-6水平均较治疗前显著降低,观察组又显著低于对照组治疗后(P<0.05);观察组CD3~+、CD4~+及CD4~+/CD8~+水平均显著高于对照组治疗后(P<0.05)。观察组不良反应发生率5.17%,显著低于对照组的13.8%(P<0.05)。结论:炎琥宁治疗小儿重症上呼吸道感染,可缩短病程,降低血清TNF-α及IL-6水平,改善免疫功能,不良反应发生率低。
Objective: To observe and compare the efficacy and safety of Yanhuning and Ribavirin in the treatment of pediatric severe upper respiratory tract infection. Methods: A total of 116 children with severe upper respiratory tract infection were randomly divided into observation group (58 cases) and control group (58 cases). Both groups were given routine treatment of fever, drinking water, digestible semi-liquid food or soft food, and the control group received ribavirin injection intravenously. The observation group was given Yanhuning injection intravenously. The disappearance time of symptoms of upper respiratory tract infection was compared between the two groups. Serum levels of TNF-α, IL-6, CD3 +, CD4 + and CD4 + / CD8 + Reaction occurred. Results: The disappearance time of nasal obstruction, cough and throat in observation group was significantly shorter or shorter than that of control group (P <0.05, P <0.01). There was no significant difference between the two groups in the withdrawal time and diarrhea stopping time (P> 0.05). After treatment, the levels of serum TNF-α and IL-6 in both groups were significantly lower than those before treatment (P <0.05), and the levels of CD3 +, CD4 + and CD4 + / CD8 ~ + levels were significantly higher than the control group after treatment (P <0.05). The incidence of adverse reactions in the observation group was 5.17%, which was significantly lower than that in the control group (13.8%, P <0.05). Conclusion: Yanhuning treatment of severe upper respiratory tract infection in children, can shorten the course, reduce serum TNF-α and IL-6 levels, improve immune function, the incidence of adverse reactions is low.
