树脂血液灌流联合针对性护理对尿毒症患者皮肤瘙痒的影响

目的探讨树脂灌流联合针对性护理对尿毒症患者皮肤瘙痒的影响。方法 45例伴尿毒症皮肤瘙痒的维持性血液透析患者,均在常规透析治疗的基础上联合树脂血液灌流,并给予针对性护理。比较患者治疗前、后血肌酐(Scr)、血尿素氮(BUN)、血磷(P)、甲状旁腺素(iPTH)以及皮肤瘙痒程度。结果治疗前、治疗6个月后Scr、BUN水平比较,差异无统计学意义(P>0.05),治疗6个月后iPTH、P水平均低于治疗前,差异有统计学意义(P<0.05);治疗6个月后瘙痒程度为(4.12±1.04)分低于治疗前的(8.21±1.26)分,差异有统计学意义(P<0.05)。结论对伴皮肤瘙痒尿毒症患者,在常规血液透析基础上,增加树脂血液灌流及针对性护理,能缓解皮肤瘙痒,降低甲状旁腺素和血磷水平,对肾功能影响较小。 Objective To investigate the effect of resin infusion combined with targeted nursing on pruritus of uremic patients. Methods A total of 45 patients with maintenance hemodialysis with uremic pruritus were treated with hemodialysis on the basis of conventional dialysis and given targeted care. The levels of serum creatinine (Scr), blood urea nitrogen (BUN), serum phosphorus (P), parathyroid hormone (iPTH) and pruritus in the patients before and after treatment were compared. Results Before treatment, the levels of Scr and BUN were not significantly different after 6 months of treatment (P> 0.05). After 6 months of treatment, the levels of iPTH and P were lower than those before treatment (P <0.05) ). The degree of pruritus after 6 months treatment was (4.12 ± 1.04) points lower than that before treatment (8.21 ± 1.26) points, the difference was statistically significant (P <0.05). Conclusions For hemodialysis patients with pruritus on the basis of conventional hemodialysis, hemodialysis of resin and targeted nursing can relieve skin pruritus, reduce parathyroid hormone and phosphorus levels, and have little effect on renal function.
