目的探讨早发型和晚发型两种新生儿败血症的相关危险因素以提高预防意识。方法回顾性分析2013-2015年该院治疗的126例败血症患儿的临床资料,分为EOS组(早发型)和LOS组(晚发型)分别进行临床观察并总结分析。结果EOS组胎膜早破、羊水污染、新生儿窒息、机械通气、孕妇产时发热、新生儿肺部感染明显高于LOS组(P<0.05),且Logistic分析表明以上危险因素对早发型新生儿败血症的发生有影响(P<0.05);LOS组的早产儿、低出生体重、新生儿脐部感染、喂养不当发生率高于LOS组(P<0.05),且Logistic分析表明以上危险因素对晚发型新生儿败血症的发生有影响(P<0.05)。结论早发型新生儿败血症的主要危险因素为胎膜早破、羊水污染、新生儿窒息、机械通气、孕妇产时发热、新生儿肺部感染;晚发型新生儿败血症的主要危险因素为早产儿、低出生体重、新生儿脐部感染、喂养不当;对新生儿败血症的高危因素充分认识,并采取积极有效的预防措施或者及时确诊,对于减少新生儿败血症的发生以及提高治愈率将有重要意义。
Objective To explore the related risk factors of sepsis in both early-onset and late-onset neonates to improve their prevention awareness. Methods The clinical data of 126 children with sepsis treated in our hospital from 2013 to 2015 were retrospectively analyzed. The patients were divided into two groups: EOS group (early onset type) and LOS group (late onset type), respectively. Results In the EOS group, premature rupture of membranes, amniotic fluid contamination, neonatal asphyxia, mechanical ventilation and intrapartum fever during pregnancy and neonatal pulmonary infection were significantly higher in EOS group than those in LOS group (P <0.05). Logistic analysis showed that the above risk factors had significant effect on early onset neonates (P <0.05). The incidence of premature infant, low birth weight, neonatal umbilical infection and improper feeding in LOS group was higher than that in LOS group (P <0.05), and Logistic analysis showed that the above risk factors Late onset neonatal sepsis have an impact (P <0.05). Conclusion The main risk factors of early-onset neonatal sepsis are premature rupture of membranes, amniotic fluid contamination, neonatal asphyxia, mechanical ventilation, fever during pregnancy and neonatal pulmonary infection. The main risk factors for late-onset neonatal sepsis are premature infants, Low birth weight, neonatal umbilical infection, improper feeding; high risk factors for neonatal full awareness of sepsis, and take active and effective preventive measures or timely diagnosis, to reduce neonatal sepsis and improve the cure rate will be of great significance.
