目的 探讨微型钢板内固定治疗MasonⅡ、Ⅲ型桡骨头骨折的临床疗效.方法 自2010年1月至2013年1月,采用切开复位微型钢板治疗MasonⅡ、Ⅲ桡骨头骨折患者15例,男10例,女5例;平均年龄36.4岁.MasonⅡ型4例,Ⅲ型11例.通过术后随访肘关节疼痛、运动、肌力、X线表现以及肘关节评分等进行研究.结果 15例患者术后均定期随访,时间7~ 15个月,平均11.1个月,骨折达到骨性愈合时间3.2~5.4个月,平均4.2个月.根据Broberg-Morrey评分:MasonⅡ型,优2例,良2例;MasonⅢ型,优8例,良2例,中1例.无一例患者发生桡神经损伤、桡骨头坏死等并发症,提携角无明显变化,其中1例患者有轻度骨化性肌炎.结论 微型钢板治疗MasonⅡ、Ⅲ型桡骨头骨折疗效满意,可获得比较满意的肘关节活动度,肌力恢复良好,并发症少,值得临床应用.“,”Objective To evaluate the clinical outcomes of treating Mason type Ⅱ and type Ⅲ radial head fractures with mini-plate internal fixation.Methods From January 2010 to January 2013,15 cases of Mason type Ⅱ and Ⅲ radial head fractures were treated with open reduction and internal fixation using miniplates.There were 10 males and 5 females with an average age of 36.4 years.Among them 4 patients had Mason type Ⅱ fractures and 11 had Mason type Ⅲ fractures.The outcomes were evaluated by assessing pain, range of motion, muscle strength, X-ray findings and elbow function rating score.Results All 15 patients were follow-up for 7 to 15 months with an average of 11.1 months.Bone healing time ranged from 3.2 to 5.4 months with an average of 4.2 months.According to the Broberg-Morrey elbow performance score, the results were rated as excellent in 2 and good in 2 of the Mason type Ⅱ fractures, and as excellent in 8, good in 2 and fair in 1 of the Mason type Ⅲ fractures.None of the patients had complications of radial nerve injury or radial head necrosis.There was no obvious change in the elbow carrying angle.Only one patient had mild myositis ossificans.Conclusion Mini-plate internal fixation for treatment of Mason type Ⅱ and Ⅲ radial head fractures can achieve satisfactory elbow motion and good muscle strength with very few complications.This procedure is worthy of clinical application.
