目的 为了解新生儿缺氧缺血性脑病( H I E) 对患儿远期体格发育和智力发育的影响。方法 对48 例经头颅 C T及临床表现确诊为 H I E 的患儿进行了远期随访,体格发育用 Kaup 指数对小儿身高、体重等进行评估,智力发育测量用 Gesell 智能发育量表。结果 H I E 与正常儿对照组在体格发育上无显著性差异( P > 0 .05) ,智力低下可发生在中、重度 H I E 患儿中。结论 降低 H I E 所致的智残,除建立自主呼吸≤20 分钟外,还要重视复苏后72 小时内保护心、肾、脑、以防止 H I E。
Objective To understand the impact of neonatal hypoxic-ischemic encephalopathy (H IE) on the long-term physical development and mental development of children. Methods Forty-eight children who had been diagnosed as H I E by cranial CT and clinical manifestations were followed up for a long time. The Kaup index was used to evaluate the height and weight of children in physical development. The Gesell intelligence development scale was used to measure the mental development. Results There was no significant difference in physical development between H I E and control group (P> 0.05). Mental retardation could occur in moderate and severe HIE patients. Conclusions To reduce the mental retardation caused by H I E, besides establishing spontaneous respiration ≤ 20 minutes, we should also pay attention to protecting the heart, kidney and brain within 72 hours after resuscitation so as to prevent H I E.
