目的 评价99mTc-MIBI脑肿瘤断层显像监测立体定向适形放疗治疗脑肿瘤的疗效。方法 对49例脑肿瘤患者进行立体定向适形放疗前后99mTc-MIBI 脑显像以及血清唾液酸(SA)测定,比较两者结果并评价临床意义。结果 治疗前49例患者中,99mTc-MIBI显像的敏感度为93.9%。治疗前后肿瘤/非肿瘤比平均值分别为9.5±7.6、2.9±5.1(P<0.001)。治疗前SA为0.245±0.038 A,其中阳性36例,诊断敏感度为73.5%;治疗后SA为0.176±0.024 A,其敏感度和特异性分别为77.8%和63.6%。治疗前99mTc-MIBI显像阴性的3例中,SA测定值阳性者2例;治疗后99mTc-MIBI显像19例阴性中,有6例SA测定值出现假阳性。结论 99mTc-MIBI脑显像诊断脑肿瘤的特异性较高,结合SA测定可提高敏感度。
Objective To evaluate the therapeutic effect of 99mTc-MIBI brain tumor tomography on stereotactic conformal radiotherapy in the treatment of brain tumors. Methods 99m Tc-MIBI brain imaging and serum sialic acid (SA) were measured before and after stereotactic radiotherapy in 49 patients with brain tumors. The clinical results were compared between the two methods. Results The sensitivity of 99m Tc-MIBI imaging was 93.9% in 49 patients before treatment. The mean tumor / non-tumor ratios before and after treatment were 9.5 ± 7.6 and 2.9 ± 5.1, respectively (P <0.001). Before treatment, the SA was 0.245 ± 0.038 A, of which 36 were positive and the diagnostic sensitivity was 73.5%. The SA after treatment was 0.176 ± 0.024 A with a sensitivity and specificity of 77.8% and 63.6%, respectively. Among the 3 cases with 99mTc-MIBI imaging negative before treatment, 2 cases were positive in SA measurement. Among the 19 cases with negative 99mTc-MIBI imaging, 6 cases had false positive SA measurement. Conclusion The specificity of 99mTc-MIBI brain imaging in the diagnosis of brain tumors is high. Sensitivity can be increased by combining with SA assay.
