目的:分析高龄不孕症患者接受辅助生殖技术(ART)后妊娠丢失的危险因素。方法:选择2014年1月至2016年5月于郑州大学第二附属医院生殖医学部接受ART的年龄≥35岁的不孕症患者,根据妊娠情况分为妊娠丢失组和妊娠持续组,回顾收集临床资料和实验室结果,多因素Logistic回归分析早期妊娠丢失的危险因素。结果:新鲜周期高龄不孕症患者的早期妊娠丢失率为38.81%,与年龄<35岁患者比较,差异有统计学意义(P<0.05)。妊娠丢失组和妊娠持续组的年龄和胚胎移植数比较,差异有统计学意义(P<0.05)。Logistics回归显示,新鲜周期中年龄和胚胎移植数是危险因素。结论:对于高龄不孕症患者,提倡尽早接受ART;胚胎移植数需谨慎决定,提倡减少移植数。
Objective: To analyze the risk factors of pregnancy loss in elderly infertility patients receiving assisted reproductive technology (ART). Methods: From January 2014 to May 2016, infertility patients aged 35 or older receiving ART at Department of Reproductive Medicine, Second Affiliated Hospital of Zhengzhou University were divided into pregnancy loss group and pregnancy continuation group according to their pregnancy status. Data and laboratory results, multivariate Logistic regression analysis of early pregnancy loss risk factors. Results: The early pregnancy loss rate of fresh cycle infertility patients was 38.81%, which was significantly different from that of patients younger than 35 years old (P <0.05). There were significant differences in the age and the number of embryo transfer between the lost pregnancy group and the persistent pregnancy group (P <0.05). Logistics regression shows that age and embryo transfer numbers are risk factors in fresh cycles. CONCLUSIONS: In elderly patients with infertility, ART is advocated as early as possible; the number of embryos to be transplanted should be determined with caution to reduce the number of transplants.
