目的:探讨孕晚期女性感染B族链球菌(GBS)对新生儿的影响。方法:选取2010年7月~2013年12月在深圳市南山区人民医院产科门诊孕晚期引导分泌物培养为GBS阳性的43例孕妇和其口咽部分泌物培养为GBS阳性的12例新生儿为研究对象(研究组);并选取同期在本院产科门诊行健康检查的100例正常孕妇为对照组,追踪随访其新生儿结局。结果:研究组新生儿体重及Apgar评分明显降低,而新生儿感染率明显升高(P<0.05)。随机选取16株新生儿菌株,共分离出6个血清型,包括Ⅰa、Ⅰb、Ⅱ、Ⅲ、Ⅴ和Ⅷ型,其中Ⅱ型占68.75%,Ⅲ型占18.75%,是主要血清型。出现新生儿肺炎的4例新生儿所提取菌株均与Ⅲ型产生沉淀反应。结论:孕晚期女性感染GBS增加新生儿感染率,导致新生儿低体重及低Apgar评分,新生儿GBS感染主要血清型为Ⅱ型,应进行合理的产前筛查及预防性治疗。
Objective: To investigate the influence of GBS on the newborn infants in the third trimester of pregnancy. Methods: From July 2010 to December 2013, 43 pregnant women with guidance GBS positive for obstetric outpatient obstetrics and gynecology clinics in Shenzhen Nanshan People’s Hospital and 12 newborn infants whose oropharyngeal secretions were GBS positive (Study group); and select the same period in our hospital obstetric clinic health examination of 100 cases of normal pregnant women as control group, follow-up of their neonatal outcome. Results: Neonatal weight and Apgar score decreased significantly in the study group, while the neonatal infection rate was significantly increased (P <0.05). Sixteen neonatal strains were randomly selected and six serotypes were isolated, including Ⅰa, Ⅰb, Ⅱ, Ⅲ, Ⅴ and Ⅷ. Among them, type Ⅱ accounted for 68.75% and type Ⅲ accounted for 18.75%, which were the major serotypes. Neonatal pneumonia occurred in 4 cases of neonatal strains were isolated and type Ⅲ precipitate reaction. CONCLUSIONS: Infection of GBS during the third trimester of pregnancy increases the neonatal infection rate, resulting in low birth weight and low Apgar score. The main serotype of neonatal GBS infection is type Ⅱ, and proper prenatal screening and prophylactic treatment should be carried out.
