目的 探讨“3+1”复合超声切面(四腔心切面+左、右心室流出道切面+三血管切面)在胎儿心脏畸形筛查中的应用价值.方法 收集接受产前超声检查的孕妇4802例,均为单胎.对每一个胎儿分别扫查四腔心基本切面、左、右心室流出道切面和三血管切面,并对图像进行评价.超声诊断为CHD的胎儿及其疾病类型均经产前诊断中心或尸体检查证实.结果 4802胎中共检出CHD胎儿35胎.3种方法对CHD的检出率由低到高依次为:四腔心切面、四腔心切面+左、右流出道切面、四腔心切面+左、右流出道切面+三血管切面[68.57%(24/35),77.14%(27/35)、100.00%(35/35),P <0.05].四腔心切面+左、右心室流出道切面+三血管切面对永存左上腔及右位主动脉弓的检出率均高于四腔心切面、四腔心+左、右心室流出道切面[100.00%(3/3)、100.00%(5/5),0(0/3)、0(0/5),0(0/3)、0(0/5);P <0.05];四腔心切面+左、右心室流出道切面和“3+1”复合超声切面,对双主动脉弓的检出率相同[100.00%(1/1)、100.00%(1/1)],且均高于四腔心切面[0(0/1)、P均<0.05)].结论 “3+1”复合超声切面可快速、准确筛查胎儿CHD,具有较高临床应用价值.“,”Objective To explore the application value of“3+1”combined cardiac view(four chamber view plus left and right ventricular outflow tract view plus three vessels view)in prenatal ultrasound screening of congenital heart disease. Methods 4802 monocyesis gravidas who had received antenatal sonographic examination were collected. Every fetus underwent prenatal ul-trasound screening of congenital heart disease with“3+1”combined cardiac view and all sonograms received evaluation. The fe-tus who was ultrasound diagnosed of CHD and the CHD type would be checked by higher level prenatal screening center or autop-sy. Results 35 CHD fetuses were detected in 4802 fetuses. The detection rates for CHD of 3 methods(four chamber view,four chamber view plus left and right ventricular outflow tract view,“3+1”cardiac view)were 68.57%(24/35),77.14%(27/35), 100.00%(35/35),with significant difference ( P <0.05). The detection rates for persistent left superior vena and right aortic arch of 3 methods were 100.00%(3/3),100.00%(5/5),0(0/3),0(0/5),0(0/3),0(0/5),with significant difference( P<0.05). Four chamber view plus left and right ventricular outflow tract view and“3+1”combined cardiac view had same detec-tion rates for double aortic arches[100%(1/1),100%(1/1)],which were higher than four chamber view(0/1),with signifi-cant difference( P <0.05). Conclusion “3+1”combined cardiac view can detect CHD quickly and accurately which and has good clinical application value.
