目的 观察帕金森病患者的视觉诱发电位 (VEP)改变 ,以期对帕金森病的诊断有所帮助。 方法 对 6 9例帕金森病患者和 6 1名健康人进行VEP和统一帕金森病评定量表 (UPDRS)检查 ,并分为老年组和非老年组进行比较。 结果 (1)老年帕金森病组VEP各波的潜伏期N75、P10 0 、N135分别为 (97 3± 16 2 )ms、(12 1 6± 10 3)ms和 (15 3 2± 17 3)ms,均较老年健康对照组各波的潜伏期〔分别为 (79 8± 15 4)ms、(10 9 6± 8 5 )ms和 (143 7± 15 8)ms〕明显延长 (均为P <0 0 1) ;非老年帕金森病组VEP各波的潜伏期N75、P10 0 、N135分别为 (96 8± 15 9)ms、(12 0 8± 9 8)ms和 (15 1 9± 16 4)ms,也比非老年健康对照组各波的潜伏期〔分别为 (78 9± 15 1)ms、(10 6 7± 7 6 )ms和 (141 9± 14 7)ms〕显著延长 (均为P <0 0 1) ;(2 )帕金森病患者VEP的P10 0 潜伏期与UPDRS评分及病程均呈正相关 ;(3)帕金森病老年组和非老年组的UPDRS评分及VEP各波的潜伏期之间差异均无显著性 (均为P >0 0 5 )。 结论 VEP作为一种电生理检查可客观地反映帕金森病患者视觉通路内多巴胺变化所致的视觉通路的电生理改变 ,对帕金森病的诊断有一定的意义。
Objective To observe the changes of visual evoked potential (VEP) in patients with Parkinson’s disease in order to diagnose Parkinson’s disease. Methods Sixty-nine patients with Parkinson’s disease and 61 healthy people were examined by VEP and Uniform Parkinson’s Disease Rating Scale (UPDRS), and divided into the elderly group and the non-elderly group for comparison. Results (1) The latent periods of VEP wave in senile Parkinson’s disease group were (97 3 ± 16 2) ms, (121 6 ± 10 3) ms and (15 3 2 ± 17 3) ms respectively (79 8 ± 15 4 ms, (10 9 6 ± 8 5) ms and (143 7 ± 15 8) ms] were significantly longer than those in the healthy control group (P <0 0 1). The latent periods of VEP wave in non-elderly Parkinson’s disease group were (96 8 ± 15 9) ms, (12 0 8 ± 9 8) ms and (15 1 9 ± 16 4) ms were also significantly longer than those in non-elderly healthy subjects (78 9 ± 15 1 ms, (10 6 7 ± 7 6) ms and (141 9 ± 14 7) ms, respectively) (P <0.01). (2) The P10 0 latency of VEP in Parkinson’s disease patients was positively correlated with the UPDRS score and duration of disease. (3) The UPDRS scores and latency of VEP waves in Parkinson’s disease elderly and non-elderly patients There was no significant difference (all P> 0.05). Conclusion As an electrophysiological examination, VEP can objectively reflect the electrophysiological changes of visual pathway induced by dopaminergic changes in the visual pathway of Parkinson’s disease patients, which is of significance for the diagnosis of Parkinson’s disease.
