目的分析脑型肝豆状核变性(HLD)患者脑深部灰质核团的磁敏感加权成像(SWI)相位值的改变,探讨SWI相位值在脑型肝豆状核变性中的应用价值。方法采用1.5 T MR系统对30例脑型HLD患者及30例健康志愿者进行SWI序列扫描。使用SPIN软件测量脑深部灰质核团红核、黑质、丘脑、尾状核头、苍白球、壳核的相位值,分析脑型HLD患者脑灰质核团相位值变化特点。结果与正常对照组比较,脑型HLD组患者红核相位值降低(HLD组:相位值为2044.12±26.96,正常组:2 075.18±31.53,t=3.291,P=0.005),黑质相位值降低(HLD组:相位值为1 826.79±83.21,正常组:1913.22±30.76,t=4.509,P=0.004),丘脑相位值降低(HLD组:相位值为2 047.13±9.32,正常组:2 082.32±21.82,t=1.132,P=0.287),苍白球相位值降低(HLD组:相位值为2 054.30±74.68,正常组:2 105.42±15.81,t=1.139,P=0.263),壳核相位值降低(HLD组:相位值为2 023.70±24.16,正常组:2 072.42±5.35,t=2.981,P=0.009),尾状核头相位值升高(HLD组:相位值为2 175.50±86.47,正常组:2 119.8±10.15,t=-1.943,P=0.069)。结论脑型肝豆状核变性患者红核、黑质、壳核相位值降低。SWI相位值有助于评价肝豆状核变性患者脑内铜的异常沉积。
Objective To analyze the change of magnetic susceptibility weighted imaging (SWI) phase of brain deep gray matter nuclei in patients with cerebral hepatolenticular degeneration (HLD), and to explore the value of SWI phase in the treatment of Wilson’s disease. Methods Thirty patients with brain-type HLD and 30 healthy volunteers were scanned by SWI using 1.5 T MR system. The SPIN software was used to measure the phase values of the nucleus accumbens, substantia nigra, thalamic nucleus, caudate nucleus, globus pallidus and putamen in the deep gray matter nuclei of brain. Results Compared with the normal control group, the phase of red nucleus decreased in patients with brain-type HLD (HLD group: 2044.12 ± 26.96 in the normal group, 2075.18 ± 31.53 in the normal group, t = 3.291, P = 0.005) (HLD group: the phase value was 1 826.79 ± 83.21, the normal group was 1913.22 ± 30.76, t = 4.509, P = 0.004), and the value of thalamic phase decreased (HLD group: the phase value was 2 047.13 ± 9.32 and the normal group was 2 082.32 ± 21.82, t = 1.132, P = 0.287). The phase of globus pallidus decreased (HLD group: phase value 2 054.30 ± 74.68, normal group 2 105.42 ± 15.81, t = 1.139, P = 0.263) (HLD group: phase value of 023.70 ± 24.16, normal group: 2072.42 ± 5.35, t = 2.981, P = 0.009), the caudate nucleus phase value increased (HLD group: phase value of 2 175.50 ± 86.47, normal Group: 2 119.8 ± 10.15, t = -1.943, P = 0.069). Conclusion The phase of red nucleus, substantia nigra and putamen in patients with Wilson’s disease is decreased. The SWI phase value helps to evaluate the abnormal deposition of copper in patients with hepatolenticular degeneration.
