目的:对临床上遇到的合并有虹膜缺损、瞳孔散大、瞳孔移位的无晶体眼或白内障病例,植入人工晶体的同时行瞳孔整复术,以恢复瞳孔的圆形及居中,提高术后视力并预防单眼复视、眩光等。方法:9点至3点方位的虹膜缺损及瞳孔上移者用10-0聚丙烯缝线直接在上方切口处行虹膜缝合(开放式);3点至9点方位的虹膜缺损则采用前房外缝合虹膜(闭合式)。结果:22例中术后视力0.5以上者占65%,瞳孔接近圆型占67%,瞳孔直径小于等于3mm占73%,瞳孔基本居中占67%。结论:人工晶体植入的同时进行瞳孔整复术,可以使各种原因引起的瞳孔变形恢复接近正常的形态、大小及位置;聚丙烯缝线应用于虹膜缝合,反应轻,效果好。眼科学报2001;17:96~98。
Objective: The clinical encounter with iris defect, mydriasis, pupil displacement of aphakia or cataract cases, implantation of intraocular lens at the same time pupil rectification to restore the pupil round and center to improve Postoperative vision and prevention of monocular diplopia, glare and so on. Methods: The iris suture (open) was performed at the upper incision with 10-0 polypropylene sutures at 9 o’clock to 3 o’clock in the iris defect and the pupil uprush. The iris defect at 3 o’clock to 9 o’clock was treated with anterior chamber Outer suture iris (closed). Results: The postoperative visual acuity of more than 0.5 accounted for 65%, pupil nearly round 67%, pupil diameter less than or equal to 3mm accounted for 73%, pupil basic 67%. CONCLUSION: Intraocular lens implantation combined with pupil repositioning can restore pupil deformity caused by various reasons to normal shape, size and position.
