目的探讨广西地区人群XPF基因RS744154多态性与胃腺癌发病风险及其临床病理特征的关系。方法应用Taq-Man-PCR方法对99例胃腺癌患者及284例正常人群的XPF基因RS744154多态性进行检测,并分析各基因型与胃腺癌患者临床病理特征的关系。结果 (1)XPF基因3种基因型(即XPF-GG、-GC和-CC型)在胃腺癌组与对照组中的分布差异具有统计学意义,以XPF-GG基因型为参照,其中携带XPF-CC者胃腺癌的发病风险增加(CC对应比值比OR为2.60);(2)分层分析显示,带风险基因型(即XPF-GC/CC)的男性、汉族或高龄者发病风险分别是不带风险基因型者的2.25倍、1.95和3.00倍(P<0.05);(3)XPF-GG、GC/CC基因型在肿瘤直径≤5 cm和>5 cm患者中出现频率分别为43.75%、71.64%和56.25%、28.36%,两者差异有统计学意义(χ2=7.198,P=0.007,OR=3.69);(4)XPF基因多态性与胃腺癌患者肿瘤发生部位、浸润深度、病理分级、Laurén分类、淋巴结转移和TNM分期均无相关性;(5)HP感染发生率在不同基因型胃腺癌患者中差异无统计学意义。结论 XPF基因RS744154多态性与广西人群胃腺癌发病风险相关,在男性、汉族及高龄者更为明显。癌旁胃黏膜HP感染与XPF基因RS744154多态性之间无显著相关性,同时其多态性也与影响胃腺癌预后的临床病理特征无相关性。
Objective To investigate the association between the polymorphism of XPF gene, RS744154 and the risk of gastric adenocarcinoma and its clinicopathological features in Guangxi. Methods The polymorphism of XPF gene rs744154 in 99 gastric adenocarcinoma patients and 284 normal subjects was detected by Taq-Man-PCR. The relationship between the genotypes and clinicopathological features of gastric adenocarcinoma was analyzed. Results (1) There were significant differences in the distribution of XPF genotypes (ie, XPF-GG, -GC and -CC genotypes) between gastric adenocarcinoma and control groups. XPF-GG genotype was the reference The risk of gastric adenocarcinoma increased in XPF-CC patients (CC: odds ratio (OR) was 2.60). (2) Hierarchical analysis showed that the risk of male, Han or elderly patients with risk genotype (XPF-GC / CC) (2.25-fold, 1.95- and 3.00-fold, respectively) with no risk genotype (P <0.05). (3) The frequency of XPF-GG and GC / CC genotypes in patients with tumor diameter ≤5 cm and> 5 cm were 43.75 %, 71.64%, 56.25%, 28.36%, respectively (χ2 = 7.198, P = 0.007, OR = 3.69); (4) There was no correlation between XPF gene polymorphisms and tumorigenesis, depth of invasion , Pathological grade, Laurén classification, lymph node metastasis and TNM staging. (5) The incidence of HP infection was not significantly different between patients with different genotypes of gastric adenocarcinoma. Conclusion The polymorphism of XPF gene RS744154 is associated with the risk of gastric adenocarcinoma in Guangxi population, especially in males, Han and elderly. There was no significant correlation between the HP infection of adjacent gastric mucosa and the polymorphism of XPF gene RS744154. Meanwhile, the polymorphism was also not associated with the clinicopathological characteristics of gastric adenocarcinoma.
