目的探讨脑发育性静脉异常(DVA)在高、低场强磁共振的不同影像学特点,增加对病变差异化表现的认知。方法回顾性分析我院6例低场强磁共振、21例高场强磁共振平扫及增强表现,研究其引流静脉及髓静脉的信号特点。结果 6例低场强磁共振平扫有3例异常粗大的引流静脉得以显示,其在T1WI上均为低信号,在T2WI上1例为低信号,2例为高信号,髓静脉未有显示。增强扫描6例都显示了引流静脉和髓静脉,其中2例呈典型的“水母头”征,另外4例分别显示了引流静脉和邻近的髓静脉;21例高场强磁共振平扫较粗大的引流静脉均得以显示,T1WI呈高信号3例,等信号4例,低信号14例,T2WI呈高信号9例,低信号12例。髓静脉显示16例,呈典型“水母头”征11例,T2WI均为高信号。结论在高、低场强磁共振上DVA有着不同的影像学特点:低场强磁共振需要增强扫描才能明确诊断;高场强磁共振表现更具有典型性,绝大部分病例在平扫的基础上就能确立诊断,而且检查序列多样化。
Objective To investigate the different imaging characteristics of brain developmental venous anomalies (DVA) in high and low field strength magnetic resonance and to increase their cognition of differentiated manifestations of lesions. Methods A retrospective analysis of 6 cases of low-field MRI in our hospital, 21 cases of high-field MRI plain scan and enhanced performance of its drainage veins and medullary vein signal characteristics. Results In 6 cases of low field MRI, 3 cases of abnormally enlarged drainage veins were shown, all of them showed low signal on T1WI, 1 case showed low signal on T2WI, 2 cases showed high signal and no evidence of myelination . Dilated veins and medullary veins were shown in 6 cases of enhanced scanning. Two cases showed typical “jellyfish head” sign, and the other 4 cases showed drainage veins and adjacent medial veins; 21 cases of high field magnetic resonance plain scan Coarse drainage veins were displayed, T1WI showed high signal in 3 cases, 4 cases of equal signal, low signal in 14 cases, T2WI showed high signal in 9 cases, low signal in 12 cases. Myelin vein showed 16 cases, showing typical “jellyfish head” sign in 11 cases, T2WI are high signal. Conclusions DVA has different imaging features in high and low field MR: low-field MRI requires enhanced scan to confirm the diagnosis; high-field MRI is more typical, and most of the cases are based on plain scan On the establishment of the diagnosis, but also check the sequence of diversification.
