目的探讨T2DM合并阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患者甲状腺功能的变化。方法选取OSAHS合并T2DM患者(OM组)47例,单纯T2DM患者(T2DM组)59例,健康对照者(NC组)58名,测定睡眠呼吸相关指标及甲状腺激素水平并进行相关性分析。结果 OM组促甲状腺激素(TSH)水平高于T2DM组和NC组[(2.47±1.35)vs(1.99±0.73)vs(1.75±0.62)μIU/ml],血清游离三碘甲状腺原氨酸(FT_3)水平低于T2DM组和NC组[(4.34±0.58)vs(4.59±0.64)vs(4.97±0.68)pmol/L](P<0.05)。Pearson相关分析显示,TSH与氧减指数(ODI)呈负相关(r=-0.315,P<0.05),FT_3与睡眠呼吸紊乱指数(AHI)、最低血氧饱和度(LSaO_2)呈正相关(r=0.238、0.266,P<0.05)。结论 T2DM合并中重度OSAHS对甲状腺功能的影响以TSH和FT_3水平的变化更显著,且TSH、FT_3均与睡眠呼吸暂停的缺氧程度相关。
Objective To investigate the changes of thyroid function in T2DM patients with obstructive sleep apnea-hypopnea syndrome (OSAHS). Methods Forty-seven OSAHS patients with T2DM (OM group), 59 T2DM patients (T2DM group) and 58 healthy control subjects (NC group) were enrolled in this study. Sleep-related respiratory parameters and thyroid hormone levels were determined and their correlations were analyzed. Results The levels of thyroid stimulating hormone (TSH) in OM group were significantly higher than those in T2DM group and NC group (2.47 ± 1.35 vs 1.99 ± 0.73 vs 1.75 ± 0.62 μIU / ml, respectively) ) Was lower than that in T2DM group and NC group (4.34 ± 0.58 vs 4.59 ± 0.64 vs 4.97 ± 0.68 pmol / L, P <0.05). Pearson correlation analysis showed that TSH was negatively correlated with ODI (r = -0.315, P <0.05), FT_3 was positively correlated with sleep apnea index (AHI) and LSAO_2 (r = 0.238, 0.266, P <0.05). Conclusion The effect of T2DM combined with moderate and severe OSAHS on thyroid function changes more significantly at TSH and FT_3 levels, and both TSH and FT_3 are related to the degree of hypoxia in sleep apnea.
