半世纪以来各国学者曾多方探索食管癌有关外科治疗上食管重建方法,其目的在于既能使食管癌肿达到较彻底的切除,减少术后癌肿复发,又能保留较多的胃功能。结肠代食管术无疑具有上述优点,即使在晚期癌肿不能切除者,作为转流术亦可暂时解决吞咽困难。但因该手术操作比较复杂,术后并发症及死亡率均较食管胃吻合术为高,直接影响近期疗效和威胁病人生命,故许多术者对此都具戒心。本文就我院(1959年8月至1963年4月)35例结肠代食管术后所发生的并发症及死亡原因予以分析和讨论,从中吸取教训,俾能减少并发症,进一步提高切除食管癌的疗效。
In the past half century, scholars from various countries have explored the esophageal reconstruction methods for surgical treatment of esophageal cancer. The purpose is to achieve a more complete resection of esophageal cancer, reduce postoperative recurrence of cancer, and retain more gastric function. Colonic esophageal surgery undoubtedly has the above advantages. Even if the advanced cancer is not resectable, as a bypass surgery can also temporarily solve dysphagia. However, because of the complicated operation, the postoperative complications and mortality are higher than those of esophageal anastomosis, which directly affects the immediate curative effect and threatens the patient’s life. Therefore, many surgeons are wary of this. This article analyzes and discusses the complications and causes of death in 35 cases of colonic esophageal surgery in our hospital (August 1959 to April 1963), learns lessons from them, and can reduce complications and further improve excision of esophageal cancer.
