目的探讨椎动脉椎间隙段频谱形态异常对血管病变的诊断价值。方法选择经脑卒中规范化颈部动脉筛查发现椎动脉椎间隙段频谱形态异常,并通过其他影像学检查(CTA、MRA或DSA)证实的154例病例影像资料进行回顾性分析。结果超声诊断“低速低阻”组34例,提示椎动脉起始段或开口处重度狭窄或闭塞,与CTA、MRA或DSA结果比较,诊断符合率为85.29%(29/34);“低速高阻”组44例,提示颅内段椎动脉重度狭窄,诊断符合率79.55%(35/44);“低速单峰”组76例,提示远段椎动脉闭塞,诊断符合率为88.16%(67/76)。结论椎间隙段椎动脉频谱形态异常可以提示其近段、远段病变,为快速寻找病灶部位和严重程度提供重要线索,值得推广应用。
Objective To investigate the diagnostic value of spectral abnormalities of vertebral artery intervertebral space segment in vascular lesions. Methods The spectrum of vertebral artery intervertebral disc was abnormally screened by standardization of stroke in the neck artery. The imaging data of 154 cases confirmed by other imaging examination (CTA, MRA or DSA) were retrospectively analyzed. Results The results of ultrasonography in 34 cases of low speed and low resistance group showed severe stenosis or occlusion at the beginning or opening of vertebral artery. The diagnostic coincidence rate was 85.29% (29/34) compared with CTA, MRA or DSA. Forty-four cases of low-speed and high-resistance group showed severe intracranial vertebral artery stenosis with a coincidence rate of 79.55% (35/44) and 76 cases of low speed unimodal group, indicating distal vertebral artery occlusion The rate was 88.16% (67/76). Conclusion Abnormalities of the vertebral arteries in the intervertebral disc segments may indicate the lesions in the proximal and distal segments and provide important clues for the rapid search of the lesion site and severity, which is worthy of popularization and application.
