目的探讨脑泡型包虫病边缘区域的1 H-MRS主要化合物浓度比值与DWI定量指标表观扩散系数(ADC值)的相关性。方法收集2009年7月—2013年3月新疆医科大学第一附属医院确诊为脑泡型包虫病的11例患者共23个目标病灶;应用3.0T超导磁共振仪,分别进行全脑常规MR平扫、增强扫描、多体素1 H-MRS及DWI检查,DWI梯度敏感因子b值取1 000s/mm2。测量脑泡型包虫病病灶实质区、边缘区(强化边界旁10 mm范围内)、对照区多体素1 H-MRS Cho/Cr、NAA/Cho、NAA/Cr、Lip/Cr化合物浓度比值及一一对应感兴趣区ADC值,对1 H-MRS化合物浓度比值与ADC值分别进行相关性分析。结果脑泡型包虫病病灶边缘区域的ADC值与NAA/Cho、NAA/Cr比值存在负相关(r分别为-0.89、-0.71),与Cho/Cr、Lip/Cr比值存在正相关(r分别为0.73、0.81)。结论 DWI与1 H-MRS联合应用,不仅有助于指导脑泡型包虫病外科手术,也可为进一步探讨其生物学行为提供有价值的信息。
Objective To investigate the correlation between the concentration ratio of 1 H-MRS in the marginal zone of cerebral edema and the apparent diffusion coefficient (ADC) of DWI quantitative index. Methods Totally 23 target lesions were collected from 11 patients with cystic echinococcosis diagnosed at the First Affiliated Hospital of Xinjiang Medical University from July 2009 to March 2013. Using 3.0T superconducting magnetic resonance instrument, MR scan, enhanced scan, multi-voxel 1 H-MRS and DWI examination, DWI gradient-sensitive factor b value of 1 000s / mm2. The ratio of multi-voxel 1 H-MRS Cho / Cr, NAA / Cho, NAA / Cr, and Lip / Cr compound concentrations in the control and the marginal zone And ADC values corresponding to the region of interest one by one, correlation analysis was performed between the concentration ratio of 1 H-MRS compound and the ADC value respectively. Results There was a negative correlation between ADC value and NAA / Cho ratio and NAA / Cr ratio (r = -0.89, -0.71, respectively) in the marginal zone of brain edema and positive correlation with Cho / Cr and Lip / Respectively 0.73,0.81). Conclusions The combination of DWI and 1 H-MRS can not only help guide the operation of cerebral echinococcosis, but also provide valuable information for further exploring its biological behavior.
