山东原是我国班氏丝虫病流行最严重的省份,分布于74个县、市,其中22个县、市的微丝蚴阳性率在10%以上,最高为26%。据50年代调查估计,全省约有微丝蚴阳性者和有症状的丝虫病患者500万人。淡色库蚊为主要传播媒介,在鲁南重流行区自然感染率高达37.5~51.9%。50年代末和60年代,曾在部分地区进行防治。1970年以来,在全省开展了大规模的防治,对微丝蚴阳性者给予海群生系统治疗,对感染较重的县、社,队继以全民服药或海群生药盐防治。至1983年全省平均微丝蚴阳性率由1970年的4.6%下降为0.1%,约有2/3的大队微丝蚴阳性率已降为0,其余的都在1%以下。据近年来的重点调查,蚊媒自然感染率已接近于0;在防治后出生的10岁以下儿童中未再有新感染发生,显示当地丝虫的传播已基本阻断。作者认为,海群生药盐的应用对控制和消灭丝虫病具有重要意义。此外,在血检微丝蚴的方法、下肢象皮肿和乳糜尿的治疗及丝虫病基本消灭后的监测等方面的研究也有新的进展。
Shandong was originally the most serious province of Bancroftian filariasis epidemic in 74 counties and cities, of which 22 counties, the city of microfilaria positive rate of 10% or more, up to 26%. According to the survey conducted in the 1950s, about 5 million people were found to have microfilaria positive and symptomatic filariasis in the province. Culex pipiens pallens as the main vector, natural infection rate in Lunan heavy areas as high as 37.5 ~ 51.9%. In the late 1950s and 1960s, some areas were controlled. Since 1970, large-scale prevention and treatment have been carried out in the whole province, and seafloor system therapy has been given to microfilariae positive persons. County-level, community-based, and team-infected monuments have been followed by universal medicine or Haiqun salt treatment. By 1983, the average positive rate of microfilariae in the province dropped from 4.6% in 1970 to 0.1%, and about 2/3 of the positive rates of microfilariae in the brigade were reduced to 0 and the rest were below 1%. According to the key surveys in recent years, the natural infection rate of mosquitoes is close to zero; no new infections have occurred in children under 10 years of age born after the prevention and treatment, indicating that the transmission of local filariasis has been basically blocked. The author believes that the application of sea qun salt has great significance in controlling and eradicating filariasis. In addition, there are some new developments in the methods of blood-screening microfilariae, the treatment of lower extremity edema and chyluria and the monitoring after the basic elimination of filariasis.
