目的探讨胰岛素泵联合高压氧治疗糖尿病并脑梗死的效果。方法60例糖尿病并脑梗死患者随机分为治疗组(30例)和对照组(30例),治疗组给予胰岛素泵联合高压氧治疗,对照组给予胰岛素泵治疗,于治疗前后进行神经功能缺损评分和临床疗效判定,同时在治疗前后进行血浆纤维蛋白原(Fib)、红细胞压积(HCT)检测。结果治疗后治疗组神经功能缺损评分显著低于对照组(P<0.01),2组治疗后与治疗前相比,评分均显著下降(P<0.01)。治疗组总有效率和显效率明显高于对照组,2组比较有显著性差异(P<0.01)。2组Fib和HCT治疗后与治疗前比较,均显著下降(P<0.01);治疗后治疗组与对照组比较,HCT显著下降(P<0.01)。结论胰岛素泵联合高压氧治疗可以明显降低糖尿病并脑梗死患者Fib、HCT,降低血粘度,增加缺氧组织的供氧和能量利用,改善患者神经功能。
Objective To investigate the effect of insulin pump and hyperbaric oxygen on diabetic patients with cerebral infarction. Methods Sixty patients with diabetes mellitus and cerebral infarction were randomly divided into treatment group (30 cases) and control group (30 cases). The treatment group was given insulin pump combined with hyperbaric oxygen therapy, the control group was given insulin pump treatment, and the neurological deficit score And clinical efficacy determination, at the same time before and after treatment of plasma fibrinogen (Fib), hematocrit (HCT) detection. Results After treatment, the scores of neurological deficit in the treatment group were significantly lower than those in the control group (P <0.01). The scores of the two groups were significantly lower than those before treatment (P <0.01). The total effective rate and markedly effective rate in the treatment group were significantly higher than those in the control group, with significant difference between the two groups (P <0.01). After treatment, Fib and HCT in both groups were significantly decreased compared with those before treatment (P <0.01); HCT was significantly decreased in treatment group and control group after treatment (P <0.01). Conclusion Insulin pump combined with hyperbaric oxygen therapy can significantly reduce Fib, HCT in patients with diabetes mellitus and cerebral infarction, reduce blood viscosity, increase oxygen supply and energy utilization in hypoxic tissues, and improve neurological function.
