目的观察双层筋膜覆盖Medpor耳支架一期耳再造术的临床效果。方法选择2013年1月~2016年5月在上海东方丽人医疗美容门诊部就诊的先天性小耳畸形269例,Medpor耳支架由美国PARK大学Porex外科医疗公司生产,雕刻好的外耳轮支架上覆盖游离筋膜瓣,翻转颞顶筋膜瓣覆盖整块支架,游离皮片覆盖筋膜瓣一期完成全耳廓再造术,术后2周,3个月,6个月及12个月随访,观察再造耳廓的成活情况及形态。结果 269例患者再造耳均成活,术后随访6~12个月未出现排异、过敏及其他不良反应。262例再造耳廓形态逼真,质感柔韧,立体感强,颅耳角与健耳对称,有3例出现支架外露,有4例因为瘢痕体质导致术后瘢痕增生,耳廓形态不佳。结论双层筋膜覆盖Medpor耳支架一期耳再造弥补了单层筋膜瓣覆盖Medpor支架植入质地偏硬的缺点,降低了外耳轮支架外露的缺点,是目前有效的应用Medpor耳支架一期耳再造的方法。
Objective To observe the clinical effect of double-stage fascia covering Medpor ear-bearing one-stage ear reconstruction. METHODS: From January 2013 to May 2016, 269 cases of congenital parietal malformations were treated at Shanghai Oriental Beauty Medical Clinic. Medpor ear brackets were produced by Porex Surgical Company of PARK University in the United States. The engraved outer ear brackets were covered with free Fascia flap, flip top temporoparietal flap covering the whole stent, the flap covered with fascia flap complete auricle reconstruction, after 2 weeks, 3 months, 6 months and 12 months follow-up observation Reproduce the survival of the pinna and shape. Results All the 269 patients survived after reconstruction, and no abnormalities, allergies and other adverse reactions occurred after 6 to 12 months of follow-up. 262 cases of reconstruction ear shape realistic, soft texture, strong sense of three-dimensional, cranial and ear ear symmetry, there are three cases of stent is exposed, 4 cases because of scar formation leads to postoperative scar hyperplasia, poor ear morphology. Conclusion The double-layer fascia-covered Medpor ear stent replaces the shortcomings of the single-layer fascia flap covering Medpor stent with rigid texture and reduces the shortcomings of the external ear stent. It is the first effective way to use Medpor ear stent Ear reconstruction method.
