目的了解兰州地区感染性腹泻婴幼儿中A组轮状病毒(RV)腹泻流行情况。方法 2009年1月至2014年6月在兰州大学第一医院儿科收集腹泻患儿粪便标本1 245份,采用酶联免疫吸附试验(ELISA)检测A组RV抗原,阳性标本采用逆转录-聚合酶链式反应(RT-PCR)进行基因分型。结果 1 245份标本中共检测出A组RV抗原608份,阳性率为48.84%。A组RV发病高峰为每年10月至次年2月。对608份A组RV阳性标本进行G、P分型,G分型以G3型为主,占23.68%(144/608),其次为G9、G2、G1和G4型,分别占14.14%(86/608)、7.24%(44/608)、7.07%(43/608)和0.82%(5/608)。P分型以P[8]、P[9]和P[4]型常见,分别占34.52%(165/478)、19.25%(92/478)和14.23%(68/478)。最常见的G、P型别组合为G8P[9]、G9P[8]和G3P[8],分别占25.94%(90/347)、19.02%(66/347)和17.29%(60/347);其次是G2P[4]、G1P[8]、G8P[3]和G3P[4],分别占10.09%(35/347)、7.20%(25/347)、4.61%(16/347)和4.03%(14/347)。结论 A组RV是兰州地区婴幼儿腹泻的常见病原,流行毒株呈现型的多样性变化。目前上市的A组RV疫苗适合兰州地区患儿使用,将G9型引入现有疫苗株具有重要的现实意义。
Objective To understand the prevalence of group A rotavirus (RV) diarrhea in infants with infectious diarrhea in Lanzhou. Methods From January 2009 to June 2014, 1 245 stool samples of diarrhea children were collected from the pediatric department of First Hospital of Lanzhou University. RV antigen of group A was detected by enzyme-linked immunosorbent assay (ELISA). The positive samples were detected by reverse transcription-polymerase (RT-PCR) for genotyping. Results A total of 608 antigens of group A were detected in 1 245 samples, the positive rate was 48.84%. The incidence of RV in group A was from October to February. The G and P genotypes of 608 RV positive specimens of Group A were G type, accounting for 23.68% (144/608), followed by G9, G2, G1 and G4, accounting for 14.14% (86 / 608), 7.24% (44/608), 7.07% (43/608), and 0.82% (5/608). P classification was common in P [8], P [9] and P [4], accounting for 34.52% (165/478), 19.25% (92/478) and 14.23% (68/478), respectively. The most common G and P type combinations were G8P [9], G9P [8] and G3P [8], accounting for 25.94% (90/347), 19.02% (66/347) and 17.29% (60/347) Followed by G2P [4], G1P [8], G8P [3] and G3P [4], accounting for 10.09% (35/347), 7.20% (25/347), 4.61% (16/347) and 4.03 % (14/347). Conclusion RV in Group A is a common pathogen of infantile diarrhea in Lanzhou. The prevalence of epidemic strains is variable. The currently available A group of RV vaccine is suitable for use in children in Lanzhou, the introduction of G9 into existing vaccine strains has important practical significance.
