目的探讨呼吸运动功能训练在肺癌术后患者肺功能恢复中的价值。方法选择2015年1月—2016年12月收治的手术治疗肺癌患者80例,随机分为观察组与对照组各40例。观察组于住院期间及出院后3个月给予呼吸运动康复训练,对照组实施常规护理。对比两组干预前后肺功能指标变化(FEV1、FEV1/FVC)、呼吸机支持时间及住院时间、圣乔治呼吸问卷(st George’s respiratory questionnaire,SGRQ)评分。计量资料比较采用t检验,P<0.05为差异有统计学意义。结果与干预前比较,干预后两组FEV1和FEV1/FVC均改善(均P<0.05),且观察组[(62.6±3.1)%、61.1±3.0]变化更明显(均P<0.05)。观察组呼吸机支持时间及住院时间[(2.1±0.1)、(36.9±4.6)d]短于对照组[(2.7±0.3)、(46.2±5.6)d](均P<0.05)。干预后2周及出院时两组SGRQ评分均降低,且观察组[(53.3±1.5)、(32.3±1.2)分]低于对照组[(73.6±2.3)、(44.5±2.0)分](均P<0.05)。结论呼吸运动功能训练可改善肺癌患者术后肺功能,缩短呼吸机治疗时间,提高生活质量。
Objective To investigate the value of respiratory function training in the recovery of pulmonary function in postoperative patients with lung cancer. Methods Eighty patients with lung cancer who underwent surgery from January 2015 to December 2016 were randomly divided into observation group (40 cases) and control group (40 cases). The observation group received respiratory exercise rehabilitation training during hospitalization and 3 months after discharge, and the control group received routine nursing care. The changes of pulmonary function indexes (FEV1, FEV1 / FVC), ventilator support time and hospital stay, and st George’s respiratory questionnaire (SGRQ) were compared before and after the intervention. Measurement data were compared using t test, P <0.05 for the difference was statistically significant. Results Compared with those before intervention, the FEV1 and FEV1 / FVC of both groups were improved after intervention (both P <0.05), and the changes in the observation group (62.6 ± 3.1% and 61.1 ± 3.0) were more obvious (all P <0.05). The ventilator support time and hospital stay in observation group were shorter than those in control group [(2.1 ± 0.1), (36.9 ± 4.6) d] [(2.7 ± 0.3), (46.2 ± 5.6) d], respectively. SGRQ scores were lower in both groups at 2 weeks and after discharge than those in the control group [(53.3 ± 1.5), (32.3 ± 1.2) points] (73.6 ± 2.3, 44.5 ± 2.0) All P <0.05). Conclusion Respiratory motor function training can improve postoperative lung function, shorten ventilator treatment time and improve quality of life.
