目的探讨彩色多普勒成像(CDFI)结合三维血管能量成像(3DCPA)在亚临床甲状腺功能减退症(亚甲减,SHT)诊断中的应用价值。方法使用CDFI和3DCPA技术检查亚甲减患者32例(病例组)和30例正常者(对照组)甲状腺进行观察,CDFI条件下,采集甲状腺上动脉收缩期峰值流速(PSV)阻力指数(RI)和达峰值流速时间(AT),对甲状腺上动脉和腺体内血管三维成像进行观察分析。通过勾勒感兴趣的范围,获得定量指标:平均灰阶(MG)、血管指数(VI)、血流指数(FI)和血流血管指数(VFI),进行对比分析。并对各参数与血清游离甲状腺素(FT4)、游离三碘甲状腺原氨酸(FT3)、促甲状腺激素(TSH)等甲状腺功能指标作相关分析。结果病例组CDFI及3D-PDA表现与正常组明显不同。病例组患者的MG、VI、FI、VFI、PSV及AT均明显高于对照组(P<0.05),RI则在病例组与对照组之间的差异无统计学意义。结论 CDFI结合3DCPA可以定性定量判定甲状腺腺体的回声水平和血流情况,可作为二维超声韵有益补充,对正确诊断亚甲减和指导临床治疗具有重要意义。
Objective To investigate the value of color Doppler imaging (CDFI) combined with three-dimensional vascular energy imaging (3DCPA) in the diagnosis of subclinical hypothyroidism (SHR). Methods The CDFI and 3DCPA were used to examine the thyroid in 32 patients with hypothyroidism and 30 healthy controls. The CDII was used to measure the systolic peak flow velocity (PSV) resistance index (RI) And up to the peak flow time (AT), the thyroid artery and glandular blood vessels three-dimensional imaging were observed and analyzed. Quantitative indexes such as MG, VI, FI and VFI were obtained by comparing the ranges of interest. The correlations between the parameters and serum thyroid hormone (FT4), free triiodothyronine (FT3) and thyroid stimulating hormone (TSH) were analyzed. Results The performance of CDFI and 3D-PDA in case group was significantly different from that in normal group. The cases of MG, VI, FI, VFI, PSV and AT were significantly higher than the control group (P <0.05), RI in the case group and the control group, the difference was not statistically significant. Conclusion CDFI combined with 3DCPA can qualitatively and quantitatively determine the echogenicity and blood flow of the thyroid gland. It can be a useful supplement for two-dimensional ultrasonography, which is of great significance for the correct diagnosis of methylenecretin reduction and clinical treatment.
