目的观察超声导引下穿刺置自制侧孔单腔中心静脉管引流联合无水乙醇硬化治疗巨大肝囊肿的安全性及疗效。方法超声引导下对14例巨大肝囊肿患者,穿刺置入自制侧孔的单腔中心静脉管引流,根据囊肿大小及引流量,每隔1~2 d注射无水乙醇冲洗治疗1次。当所有患者24 h引流量<10 ml时,再次硬化后拔除引流管。结果本组病例技术成功率100%,治疗有效率92.8%(13/14),治疗后6个月,复查CT示治愈12例,治愈率85.7%。并发症仅为醉酒现象、局部疼痛等,无一例并发出血、黄疸、感染或其他不良反应。结论超声导引下置自制侧孔单腔中心静脉管引流联合无水乙醇硬化治疗巨大肝囊肿疗效好、成功率高、创伤小,且简单经济。
Objective To observe the safety and curative effect of ultrasound-guided percutaneous insertion of a single lateral ventricular central venous catheter and absolute alcohol sclerotherapy in the treatment of giant hepatic cysts. Methods A total of 14 patients with giant hepatic cysts underwent ultrasound guided drainage of single-lumen central venous catheters inserted into homemade lateral holes. The patients were treated with ethanol injection once every 1 to 2 days according to the cyst size and drainage volume. When all patients underwent a drainage of less than 10 ml at 24 h, the drainage tube was hardened again. Results The success rate of this group of patients was 100%, the effective rate was 92.8% (13/14). After 6 months of treatment, 12 cases were cured by CT scan, the cure rate was 85.7%. Complications only for the phenomenon of drunkenness, local pain, no case of concurrent bleeding, jaundice, infection or other adverse reactions. Conclusion Ultrasound-guided home-made side hole single-lumen central venous drainage combined with absolute ethanol sclerosis treatment of giant hepatic cysts with good success, high success rate, less trauma, and simple and economical.
