目的探讨乳腺癌基因-1(BRCA1)和肿瘤相关抗原(CA125)在子宫内膜癌(EC)中的表达及其临床意义。方法选取2014年1月—2016年2月在德阳市人民医院就诊的EC患者56例,非典型子宫内膜增生患者32例,子宫肌瘤患者26例,采用免疫组织化学法检测BRCA1和CA125在不同子宫内膜组织中的表达,并对EC患者在不同病理因素下BRCA1和CA125的表达差异进行分析。结果 BRCA1和CA125在EC、非典型子宫内膜增生及子宫肌瘤患者子宫内膜中的阳性表达率分别为32.14%、75.00%、96.15%和69.64%、53.13%、7.69%,其中BRCA1在EC患者中的表达显著低于其他2组(P<0.05),而CA125在EC患者中的表达显著高于其他2组(P<0.05)。BRCA1的阳性表达率在手术病理分期、组织分化、淋巴有无转移的组间差异均具有统计学意义(P<0.05),但与肌层浸润程度无关;CA125的阳性表达率与不同手术病理分期、组织分化、肌层浸润有关,而与淋巴结转移无关;子宫内膜组织中BRCA1和CA125的表达呈负相关(r=-0.319,P=0.013)。结论 EC患者子宫内膜中BRCA1呈低表达,CA125呈高表达,且BRCA1和CA125在EC中的水平变化与组织分化程度、病理分期等指标有关,可用于评估EC恶性程度,在临床诊断中具有重要意义。
Objective To investigate the expression of breast cancer gene-1 (BRCA1) and tumor-associated antigen (CA125) in endometrial carcinoma (EC) and its clinical significance. Methods From January 2014 to February 2016, 56 patients with EC in Deyang People’s Hospital, 32 patients with atypical endometrial hyperplasia and 26 patients with uterine myoma were enrolled in this study. The expressions of BRCA1 and CA125 were detected by immunohistochemistry Different endometrial tissue expression, and EC patients under different pathological factors BRCA1 and CA125 expression differences were analyzed. Results The positive expression rates of BRCA1 and CA125 in EC, atypical endometrial hyperplasia and endometrial uterine fibroids were 32.14%, 75.00%, 96.15% and 69.64%, 53.13% and 7.69% The expression of CA125 in patients with EC was significantly lower than that in the other two groups (P <0.05), while the expression of CA125 in EC patients was significantly higher than that in the other two groups (P <0.05). The positive expression rate of BRCA1 was significantly different between the two groups (P <0.05), but not with the degree of myometrial infiltration. The positive expression rate of CA125 was correlated with different pathological stages , Tissue differentiation and myometrial invasion, but not with lymph node metastasis. The expression of BRCA1 and CA125 in endometrial tissue was negatively correlated (r = -0.319, P = 0.013). Conclusions The expression of BRCA1 in endometrium of EC patients is low and the expression of CA125 is high. The change of BRCA1 and CA125 levels in EC is correlated with the degree of tissue differentiation and pathological stage, which can be used to evaluate the malignancy of EC. Significance.
