目的探讨弥漫性大B细胞淋巴瘤(DLBCL)组织中核转录因子(NF-κBp65)、血管内皮生长因子(VEGF)的表达及其与微血管密度(MVD)的关系及临床意义。方法应用免疫组化法对58例DLBCL组织和10例反应增生淋巴结组织中NF-κBp65、VEGF和MVD进行检测。结果58例弥漫性大B细胞淋巴瘤组织中NF-κBp65阳性表达率为58.8%,VEGF阳性表达率为55.2%,两者在反应增生淋巴结组织中都不表达。NF-κBp65的表达与VEGF的表达呈正相关。NF-κBp65与VEGF阳性表达组MVD为35.63±10.23、34.24±10.54,高于NF-κBp65、VEGF阴性组的29.12±6.24、27.32±5.86(P﹤0.05)。NF-κBp65阳性表达与临床分期、出现全身症状、疗效及总生存期短显著相关(P﹤0.05),VEGF阳性表达与临床分期、疗效、IPI、LDH及总生存期短显著相关(P﹤0.05)。结论NF-κBp65信号通道促进VEGF高表达,影响肿瘤内MVD,与弥漫性大B细胞淋巴瘤的发生、发展关系密切。
Objective To investigate the expression of nuclear factor-kappa B (NF-κB) p65 and vascular endothelial growth factor (VEGF) in diffuse large B cell lymphoma (DLBCL) and its relationship with microvessel density (MVD) Methods The expressions of NF-κBp65, VEGF and MVD in 58 cases of DLBCL and 10 cases of reactive hyperplasia lymph nodes were detected by immunohistochemistry. Results The positive expression rate of NF-κBp65 in 58 cases of diffuse large B-cell lymphoma was 58.8% and the positive rate of VEGF was 55.2%, both of which were not expressed in the reactive hyperplasia lymph nodes. The expression of NF-κBp65 was positively correlated with the expression of VEGF. The MVD of NF-κBp65 and VEGF positive expression group was 35.63 ± 10.23,34.24 ± 10.54, which was higher than that of NF-κBp65 and VEGF negative group (29.12 ± 6.24,27.32 ± 5.86, P <0.05). The positive expression of NF-κBp65 was significantly associated with clinical stage, systemic symptoms, curative effect and overall survival (P <0.05). The positive expression of NF-κBp65 was correlated with clinical stage, efficacy, IPI, LDH and total survival ). Conclusion The NF-κBp65 signaling pathway promotes the high expression of VEGF and affects the intra-tumor MVD, which is closely related to the occurrence and development of diffuse large B-cell lymphoma.
