目的探讨上皮型钙黏蛋白(E-cadherin,E-cad)及其与连环蛋白复合体(cadherincatenincomplex,CCC)的表达与垂体腺瘤侵袭性的关系。方法垂体腺瘤78例,按Cottier等提出的鞍区MRI影像学判定标准将其分为侵袭性垂体腺瘤和非侵袭性垂体腺瘤。其中侵袭性垂体腺瘤44例,非侵袭性垂体腺瘤34例。免疫组织化学SP法检测E-cad及其复合体在侵袭性垂体腺瘤和非侵袭性垂体腺瘤中的表达。分析E-cad及其复合体的表达与垂体腺瘤激素分泌、是否伴有瘤卒中、坏死、囊性变及肿瘤大小等生物学行为的关系。结果44例侵袭性垂体腺瘤中30例(68·2%)表现为E-cad、α-cat和β-cat一致性低表达,14例(31·8%)表现为E-cad、α-cat和β-cat高、低表达特征无一致性。E-cad、α-cat在侵袭性垂体腺瘤和非侵袭性垂体腺瘤之间的表达差异有统计学意义(χ2=13·969,P<0·01);β-cat在侵袭性垂体腺瘤的表达较非侵袭性垂体腺瘤表达低,但差异无统计学意义(χ2=0·430,P>0·05);E-cad、α-cat在大腺瘤组较微腺瘤组表达低,两组之间差异有统计学意义(χ2=5·038,P<0·05)。E-cad在无功能垂体腺瘤较激素分泌性垂体腺瘤表达低,两组之间差异有统计学意义(χ2=4·614,P<0·05)。β-cat在伴有瘤卒中、坏死、囊性变组较不伴有组表达低,两组之间差异有统计学意义(χ2=6·701,P<0·05)。结论E-cad及其复合体的低表达或表达缺失与垂体腺瘤的侵袭性有关。
Objective To investigate the relationship between the expression of E-cadherin and its relationship with the expression of catenin complex (CCC) and the invasiveness of pituitary adenomas. Methods Seventy-eight cases of pituitary adenoma were divided into invasive pituitary adenoma and non-invasive pituitary adenoma according to the sellar MRI imaging criteria proposed by Cottier et al. Among them, 44 cases were invasive pituitary adenomas and 34 cases were non-invasive pituitary adenomas. Immunohistochemical SP method was used to detect the expression of E-cad and its complex in invasive pituitary adenomas and non-invasive pituitary adenomas. The relationship between the expression of E-cad and its complex and the secretion of hormone in pituitary adenomas was associated with biological behaviors such as stroke, necrosis, cystic degeneration and tumor size. Results Thirty (68 · 2%) of 44 invasive pituitary adenomas showed low expression of E-cad, α-cat and β-cat, 14 (31.8%) showed E-cad, -cat and β-cat high, low expression characteristics of no consistency. The expression of E-cad, α-cat in invasive pituitary adenomas and non-invasive pituitary adenomas was significantly different (χ2 = 13.969, P <0.01) The expression of adenoma was lower than non-invasive pituitary adenoma, but the difference was not statistically significant (χ2 = 0.43, P> 0.05) Group expression was low, the difference between the two groups was statistically significant (χ2 = 5 · 038, P <0 · 05). The expression of E-cad in non-functioning pituitary adenomas was lower than that in hormone-secreting pituitary adenomas (χ2 = 4.146, P <0.05). β-cat was associated with tumor stroke, necrosis, cystic degeneration group less associated with low expression group, the difference between the two groups was statistically significant (χ2 = 6 · 701, P <0 · 05). Conclusion The low expression or loss of E-cad and its complex is related to the invasiveness of pituitary adenoma.
