目的评价~(18)F-FDG PET/CT和增强MSCT诊断胰腺癌的价值。方法计算机检索Pub Med、EMBASE、The Cochrane library、Web of Science、中国生物医学文献数据库、万方数字化期刊数据库和中国期刊全文数据库等,检索时间截止2015年3月。查找有关PET/CT及增强MSCT对胰腺癌的诊断性试验。由2位评价员按照纳入与排除标准独立筛选文献、提取资料和评价纳入研究的方法学质量后,采用Meta-Disc 1.4软件进行Meta分析。结果最终纳入11篇文献,包括506例患者。以手术或组织活检病理、临床及其他影像随访为金标准,PET/CT和增强CT诊断胰腺癌的合并敏感度、特异度及SROC曲线下面积AUC值分别为0.93[95%CI(0.89,0.95)]、0.76[95%CI(0.67,0.83)]、0.9149和0.80[95%CI(0.75,0.84)]、0.66[95%CI(0.57,0.75)]、0.8159;PET/CT结合增强CT的合并敏感度、特异度及SROC曲线下面积AUC值分别为0.98[95%CI(0.94,0.99)]、0.90[95%CI(0.81,0.96)]、0.9873。结论 18F-FDG PET/CT诊断胰腺癌的敏感度较增强MSCT高,特异度无显著差异,但将二者结合检查可进一步提高诊断准确性。
Objective To evaluate the value of ~ (18) F-FDG PET / CT and enhanced MSCT in the diagnosis of pancreatic cancer. Methods Pub Med, EMBASE, The Cochrane library, Web of Science, Chinese Biomedical Literature Database, Wanfang Digital Periodical Database and Chinese Journal Full-text Database were searched by computer. The search time was up to March 2015. Find out about PET / CT and enhanced MSCT diagnostic test of pancreatic cancer. Meta-Disc 1.4 software was used for Meta-analysis after two reviewers independently screened the literature according to inclusion and exclusion criteria, extracted data and evaluated the quality of the methodology included in the study. The results eventually included 11 articles, including 506 patients. The combined sensitivity, specificity and area under the SROC curve of PET / CT and enhanced CT in the diagnosis of pancreatic cancer were 0.93 [95% CI (0.89, 0.95 ), 0.76 [95% CI (0.67,0.83)], 0.9149 and 0.80 [95% CI (0.75,0.84)], 0.66 [95% CI (0.57,0.75)], 0.8159; The combined sensitivity, specificity, and area under the SROC curve were 0.98 [95% CI (0.94,0.99)], 0.90 [95% CI (0.81,0.96)], and 0.9873, respectively. Conclusion The sensitivity of 18F-FDG PET / CT in the diagnosis of pancreatic cancer is higher than that of enhanced MSCT. There is no significant difference in the specificity of 18F-FDG PET / CT. However, the combination of both can further improve the diagnostic accuracy.
