鼻窦真菌感染在世界大部分地区均有报告,尤其是干热地带。促使其发病的因素有糖尿病和免疫缺陷,因鼻窦炎而反复应用抗生素亦为可能的诱发因素。本文报告二例伴有颅底损害的患者亦符合上述特点。患者男、女各一,分别为伊拉克及巴基斯坦籍。有长期慢性鼻窦炎或鼻炎并发鼻息肉史,并曾为此反复使用过抗生素药物。二例均出现中枢或脑神经受侵的症状与体征。颅骨X线平片和断层片均显示上颌窦、筛窦、蝶窦密度增高,鞍底有骨质侵蚀。CT扫描分别显示鞍上肿块和小脑脑桥角肿块。二例均经蝶窦活检,组织中都找到菌丝;例一为藻菌目菌丝,例二为曲霉菌菌丝。
Sinus fungal infections are reported in most parts of the world, especially in dry and hot areas. Factors that contribute to its pathogenesis are diabetes and immunodeficiency, and repeated use of antibiotics due to sinusitis is also a possible predisposing factor. This article reports two patients with skull base injury also meet the above characteristics. Patients, male and female, respectively, Iraq and Pakistani nationality. There is a long history of chronic sinusitis or rhinitis with nasal polyps, and antibiotics have been used repeatedly for this purpose. In both cases, symptoms or signs of central or cranial nerve involvement were found. X-ray skull and tomographic films showed the maxillary sinus, ethmoid sinus, sphenoid sinus density increased, there is bone erosion in the sella. The CT scan showed the suprasellar bump and the cerebellopontine angle bump respectively. Two cases were via the sphenoid sinus biopsy, mycelium found in the organization; one case of algae mycelium, case two aspergillus mycelium.
