目的探讨血清脂质结合唾液酸(LASA)、组织多肽抗原(TPA)、组织蛋白酶(CATL)、卵巢癌抗原X1(OVX1)及CA125联检对卵巢上皮性癌(EOC)的早期诊断价值。方法选择2007年8月至2010年11月兰州大学第二医院和兰州市第一人民医院妇产科拟行手术治疗的卵巢上皮性癌患者86例及良性上皮性卵巢肿瘤患者100例、健康对照组50例,测定其LASA、TPA、CATL、OVX1血清水平。并收集相应卵巢肿瘤患者的临床资料,分析其CA125水平。结果 5项指标恶性组均明显高于良性组和健康对照组(P均<0.05)。TPA对EOC的总阳性率以及早期检测阳性率均最高,LASA和OVX1对EOC的总阳性率均低于CA125,CATL对EOC的总阳性率略高于CA125,但是对EOC早期检测阳性率均高于CA125。CA125+TPA对EOC的总阳性率以及早期检测阳性率也均为最高;余3项指标与CA125两两联合检测时对EOC早期检测阳性率均明显高于CA125单检(P均<0.05),对EOC的总阳性率均明显高于CA125单检,除CA125+OVX1与CA125单检时差异无统计学意义外,其余差异均有统计学意义(P均<0.05)。结论血清LASA、TPA、CATL及OVX1是具有潜力的EOC诊断指标,TPA和CA125联检是理想的组合方式,兼顾了对EOC的总阳性率以及早期检测阳性率,提高了对EOC的诊断率。
Objective To investigate the value of serum lipids-associated sialic acid (LASA), tissue polypeptide antigen (TPA), cathepsin (CATL), ovarian cancer antigen X1 (OVX1) and CA125 in the early diagnosis of epithelial ovarian cancer (EOC) Methods Eighty-six patients with ovarian epithelial carcinoma undergoing surgery and 100 patients with benign epithelial ovarian tumors underwent surgery from the Second Affiliated Hospital of Lanzhou University and the First People’s Hospital of Lanzhou from August 2007 to November 2010 were enrolled in this study. Group of 50 cases, measured serum LASA, TPA, CATL, OVX1 levels. The clinical data of patients with ovarian tumors were collected and their CA125 levels were analyzed. Results The five indexes in malignant group were significantly higher than those in benign group and healthy control group (all P <0.05). The total positive rate of EOC and the positive rate of early detection of TPA were the highest. The total positive rate of EOC of LASA and OVX1 was lower than that of CA125. The total positive rate of CAT and EOC was slightly higher than that of CA125, but the positive rate of early EOC At CA125. The positive rate of EOC and the positive rate of early detection of CA125 + TPA were also the highest. The positive rate of early detection of EOC in CA125 + TPA and CA125 was significantly higher than that in CA125 (P <0.05) The positive rate of EOC was significantly higher than that of CA125 single test, except for the difference between CA125 + OVX1 and CA125 alone was not statistically significant, the other differences were statistically significant (P all <0.05). Conclusions Serum LASA, TPA, CATL and OVX1 are potential diagnostic indicators for EOC. The combination of TPA and CA125 is the ideal combination method, which takes into account the total positive rate of EOC and the positive rate of early detection, and improves the diagnostic rate of EOC.
