目的探讨2型糖尿病(T2DM)患者颈动脉内中膜厚度(CIMT)与血清成纤维细胞生长因子21(FGF21)水平的相关性。方法 84例T2DM患者分为CIMT正常组(A组,CIMT<0.9mm,42例)和CIMT增厚组(B组,CIMT≥0.9mm,42例),另选健康体检者40例作为对照(C组)。ELISA法检测血清FGF21浓度,分析CIMT与各临床指标的相关性。结果 A、B组血清FGF21水平高于C组(P<0.01),B组FGF21水平亦高于A组(P<0.01)。CIMT与FGF21、腰围、腰臀比、HbA1c、FBG、HOMA-IR和TG呈正相关(r=0.323、0.316、0.347、0.412、0.406、0.287、0.258,P<0.01或P<0.05),与HDL-C呈负相关(r=-0.362,P<0.01)。FBG、腰围和FGF21为CIMT的独立影响因素(P<0.05或P<0.01)。结论 CIMT增加的T2DM患者脂肪因子FGF21水平显著升高,可能在T2DM动脉粥样硬化形成中起作用。
Objective To investigate the relationship between carotid intima-media thickness (CIMT) and serum fibroblast growth factor 21 (FGF21) levels in type 2 diabetes mellitus (T2DM). Methods Eighty-four patients with T2DM were divided into two groups: control group (CIMT <0.9mm, n = 42) and CIMT thickened group (group B, CIMT≥0.9mm, Group C). The concentration of serum FGF21 was detected by ELISA, and the correlation between CIMT and various clinical indexes was analyzed. Results The levels of serum FGF21 in group A and group B were significantly higher than those in group C (P <0.01). The level of FGF21 in group B was also significantly higher than that in group A (P <0.01). CIMT was positively correlated with FGF21, waist circumference, waist-hip ratio, HbA1c, FBG, HOMA-IR and TG (r = 0.323,0.316,0.347,0.412,0.406,0.287,0.258, P <0.01 or P < C was negatively correlated (r = -0.362, P <0.01). FBG, waist circumference and FGF21 were independent factors of CIMT (P <0.05 or P <0.01). Conclusions The level of adipokine FGF21 in T2DM patients with increased CIMT is significantly increased, which may play a role in the atherosclerosis of T2DM.
