目的探讨血清C反应蛋白(CRP)/高分子量脂联素(HMWA)比值与代谢综合征(MS)的相关性,分析CRP/HMWA比值作为一种新的生物学标志物在预测MS过程中与单独使用CRP、HMWA相比是否具有优势。方法共纳入研究对象170例,根据2009年国际多学会联合声明(JIS)的MS诊断标准分为MS组(n=94)和非代谢综合征(non-MS)组(n=76);所有研究对象均行口服葡萄糖耐量试验(OGTT)及胰岛素释放试验。采用ELISA法测定血清HMWA浓度,采用化学发光法测定血清CRP浓度。结果 MS组血清CRP/HMWA比值水平高于non-MS组(P<0.01)。简单相关分析显示,血清CRP/HMWA水平与体质指数(BMI)、腰围(WC)、腰臀比(WHR)、收缩压(SBP)、空腹血糖(FPG)、OGTT 2 h血糖(2h PG)、空腹胰岛素(FIns)、OGTT 2 h胰岛素(2h Ins)、糖化血红蛋白(Hb A1c)、三酰甘油(TG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、稳态模型胰岛素抵抗指数(HOMA-IR)、CRP呈正相关(P<0.05或P<0.01),与高密度脂蛋白胆固醇(HDL-C)、胰岛β细胞功能(HOMA-β)、HMWA呈负相关(P<0.01)。Logistic回归显示,血清CRP/HMWA升高是MS的危险因素(P<0.05)。高CRP低HMWA组MS检出率高于低CRP高HMWA组(P<0.01)。血清CRP/HMWA预测MS的ROC曲线下面积高于CRP及HMWA(P<0.05)。结论与血清CRP、HMWA相比,用血清CRP/HMWA预测MS更有价值。
Objective To investigate the correlation between serum C-reactive protein (CRP) / high molecular weight adiponectin (HMWA) ratio and metabolic syndrome (MS), and to analyze the relationship between CRP / HMWA ratio as a new biomarker in predicting MS Whether CRP alone or HMWA is advantageous. METHODS: A total of 170 study subjects were enrolled in this study. They were divided into MS group (n = 94) and non-MS group (n = 76) according to the diagnostic criteria of MS in 2009, Subjects underwent oral glucose tolerance test (OGTT) and insulin release test. The concentration of serum HMWA was determined by ELISA and the concentration of serum CRP was determined by chemiluminescence. Results The serum CRP / HMWA ratio in MS group was higher than that in non-MS group (P <0.01). Simple correlation analysis showed that serum CRP / HMWA levels were positively correlated with BMI, WHR, SBP, FPG, 2h PG, (FIns), OGTT 2h Ins, Hb A1c, TG, TC, LDL-C, homeostasis model insulin (P <0.05 or P <0.01), but negatively correlated with HDL-C, HOMA-β and HMWA (P <0.01) 0.01). Logistic regression showed that elevated serum CRP / HMWA was a risk factor for MS (P <0.05). The detection rate of MS in high CRP low HMWA group was higher than that in low CRP high HMWA group (P <0.01). Serum CRP / HMWA predicted that the area under the ROC curve of MS was higher than CRP and HMWA (P <0.05). Conclusion Serum CRP / HMWA is more valuable for predicting MS than serum CRP and HMWA.
