目的探讨T2DM合并冠心病(CHD)患者血浆总同型半胱氨酸(tHcy)及脂蛋白a(Lp-a)水平的变化及其与胰岛素抵抗的相关性。方法 T2DM患者62例,分为并发CHD组及未并发CHD(CHD-)组,健康体检者45例为对照(NC)组,检测血浆tHcy、Lp-a、FPG、FIns,计算胰岛素敏感性指数(ISI),并进行相关性分析。结果 DM+CHD组tHcy、Lp-a、FIns、FPG均显著高于、ISI显著低于CHD—组和NC组(P<0.01或P<0.05)。DM+CHD组ISI与tHcy、Lp-a呈负相关,CHD-组无显著相关性。结论糖尿病检测tHcy、Lp-a水平对于了解患者病情、指导临床治疗和预防糖尿病大血管并发症有一定意义。
Objective To investigate the changes of plasma total homocysteine (tHcy) and lipoprotein (a) (Lp-a) in patients with T2DM complicated with coronary heart disease (CHD) and its relationship with insulin resistance. Methods Totally 62 patients with T2DM were divided into two groups: CHD group and CHD group, and 45 healthy control subjects as NC group. Plasma tHcy, Lp-a, FPG and FIns were measured and insulin sensitivity index (ISI), and conduct correlation analysis. Results The levels of tHcy, Lp-a, FIns and FPG in DM + CHD group were significantly higher than those in CHD group and NC group (P <0.01 or P <0.05). There was a negative correlation between ISI and tHcy, Lp-a in DM + CHD group, but no significant correlation in CHD-group. Conclusion The detection of tHcy and Lp-a in diabetes mellitus is of certain significance in understanding the patient’s condition, guiding clinical treatment and preventing diabetic macrovascular complications.
