目的通过探讨新生儿听力筛查中假阳性发生的原因,降低假阳性的发生率。方法使用丹麦AC-CUSREEN手持式耳声发射仪,对我院2008年1月~2008年12月47例未通过筛查的新生儿进行筛查情况的分析。结果1350例进行筛查的新生儿中有47例未通过筛查,其中双耳未通过18例,占1.3%,一侧未通过29例,占2.2%。经过跟踪回访,有1例早产儿3个月后复查仍未通过,经诊断确诊为听力障碍儿;1例系右耳道闭锁;1例其父母均为聋哑患者,新生儿经ABR听性测试诊断为听力障碍。结论新生儿听力筛查假阳性的发生有多方面的原因,采取环境噪音的控制、测试时机的选择、测试探头的放置、专业人员的培训都可以使假阳性的发生率显著降低。
Objective To explore the causes of false positives in neonatal hearing screening to reduce the incidence of false positives. Methods Using the Danish AC-CUSREEN hand-held otoacoustic transmitter, we analyzed the screening results of 47 newborns who did not pass screening in our hospital from January 2008 to December 2008. Results Of the 1350 newborns screened, 47 were not screened. Among them, 18 did not pass through the ears, accounting for 1.3% of the ears, while 29 failed on the one side, accounting for 2.2%. After a follow-up visit, 1 case of premature children 3 months after the review has not yet passed, diagnosed as hearing-impaired children; 1 case of right ear atresia; 1 case of their parents are deaf-mute, neonatal ABR after hearing Test diagnosed as hearing impairment. Conclusion There are many reasons for the false positive of neonatal hearing screening. The control of environmental noise, the selection of test timing, the placement of test probes and the training of professional staff can all significantly reduce the incidence of false positives.
