目的评价99Tcm-HL91在乳腺良恶性肿瘤鉴别诊断中的价值。方法临床疑乳腺肿瘤35例,静脉注射99Tcm-HL91925MBq后分别于1、3、6 h行乳腺显像,观察影像特点并进行半定量分析。所有病例于显像后1周内手术并行病理检查。结果经病理证实,24例阳性病例中有23例为恶性肿瘤,假阳性1例,11例阴性病例均为良性肿瘤,灵敏度100%,特异性91.7%,符合率97.1%。病灶与对侧相应部位放射性计数对比,恶性肿瘤比值为1.37±0.21;良性肿瘤为0.99±0.05,有显著性差异。恶性肿瘤中1 h计数比为1.31±0.02,3 h为1.34±0.16,6 h为1.45±0.29,三者之间无显著性差异。结论乳腺肿瘤99Tcm-HL91显像在乳腺肿瘤良恶性鉴别诊断中具有较高的临床应用价值。
Objective To evaluate the value of 99Tcm-HL91 in the differential diagnosis of benign and malignant breast tumors. Methods Thirty-five patients with suspected breast tumors were treated with intravenous injection of 99Tcm-HL91925MBq, respectively. The images of the breasts were observed at 1, 3, and 6 h after the intravenous injection. The imaging features were observed and semi-quantitative analysis was performed. All cases were performed pathological examination within 1 week after imaging. Results The pathological examination showed that 23 of 24 positive cases were malignant tumors, 1 was false positive and 11 negative were benign tumors with sensitivity of 100%, specificity of 91.7% and coincidence rate of 97.1%. The radioactive counts of the corresponding lesion and the contralateral counterpart were compared, the ratio of malignant tumor was 1.37 ± 0.21; the benign tumor was 0.99 ± 0.05, with significant difference. The ratio of 1 h in malignant tumors was 1.31 ± 0.02, the average time was 1.34 ± 0.16 in h, 1.45 ± 0.29 in h, and no significant difference was found between the three. Conclusion 99Tcm-HL91 imaging of breast tumor has high clinical value in differential diagnosis of benign and malignant breast tumors.
