目的探讨环氧合酶2(COX-2)和P-糖蛋白(P-gp)在肝细胞癌(HCC)中的表达及与临床病理特征的关系。方法应用免疫组织化学法检测116例HCC组织和癌旁组织COX-2和P-gp表达,分析其与临床病理特征的关系。结果 COX-2在HCC组织中的高表达率明显低于癌旁组织(61.2%vs.98.3%)(P<0.01),在中高分化组中的高表达率明显高于低分化组(69.5%vs.52.6%)(P<0.05),在甲胎蛋白(AFP)增高组中的高表达率明显低于AFP正常组(55.1%vs.73.7%)(P<0.05)。P-gp在HCC组织中的高表达率明显高于癌旁组织(50.0%vs.3.5%)(P<0.01),在合并有肝硬化组中的高表达率明显低于无肝硬化组(42.3%vs.65.8%)(P<0.05)。结论 COX-2在HCC组织中具有较高表达,是反应HCC生物学行为的有效指标,对HCC的发生发展可能具有一定作用,有望成为HCC基因治疗的靶点。
Objective To investigate the expression of cyclooxygenase 2 (COX-2) and P-glycoprotein (P-gp) in hepatocellular carcinoma (HCC) and its relationship with clinicopathological features. Methods The expressions of COX-2 and P-gp in 116 cases of HCC tissues and adjacent normal tissues were detected by immunohistochemistry, and their relationship with clinicopathological features was analyzed. Results The high expression rate of COX-2 in HCC tissues was significantly lower than that in paracancer tissues (61.2% vs.98.3%, P <0.01). The high expression rate of COX-2 in HCC tissues was significantly higher than that in poorly differentiated tissues (69.5% vs.52.6%) (P <0.05). The high expression rate in AFP group was significantly lower than that in AFP normal group (55.1% vs.73.7%, P <0.05). The high expression rate of P-gp in HCC tissues was significantly higher than that in adjacent non-cancerous tissues (50.0% vs.3.5%, P <0.01). The high expression rate of P-gp was significantly lower in patients with cirrhosis than in those without cirrhosis 42.3% vs.65.8%) (P <0.05). Conclusions COX-2 is highly expressed in HCC tissues and is an effective indicator of biological behavior of HCC. It may play a role in the development of HCC and is expected to be the target of HCC gene therapy.
