目的探讨腹腔镜探查在胃肠外科晚期和不明性质肿瘤诊治中的作用。方法分析胃肠道晚期和不明性质肿瘤104例患者的临床资料。随机分为腹腔镜探查(A组,53例)和剖腹探查(B组,51例)。对比两组病死率,并发症发生率,住院天数。结果A组病死率为0,并发症发生率为15.09%;B组病死率为11.76%,并发症发生率为43.14%,两指标A,B组间差异均有统计学意义(P<0.05)。A组住院时间为(7.25±1.65)d,显著少于B组的(18.72±4.94)d(P<0.01)结论腹腔镜探查对胃肠外科晚期和不明性质肿瘤病例的诊治具有明显优势,可在微小创伤下探明病情、取得病理诊断并予适当手术处理,可降低晚期肿瘤的手术风险、并发症率和病死率,缩短住院时间。
Objective To investigate the role of laparoscopy in the diagnosis and treatment of advanced and unknown gastrointestinal tumors. Methods Clinical data of 104 patients with advanced and unknown gastrointestinal cancers were analyzed. Randomly divided into laparoscopic exploration (A group, 53 cases) and laparotomy (B group, 51 cases). Compare the two groups of mortality, the incidence of complications, hospital days. Results The mortality of group A was 0 and the incidence of complication was 15.09%. The mortality of group B was 11.76% and the complication rate was 43.14%. There was significant difference between group A and group B (P <0.05) . The length of hospital stay in group A was (7.25 ± 1.65) d and significantly less than that in group B (18.72 ± 4.94) d (P <0.01). Conclusions Laparoscopy has obvious advantages in the diagnosis and treatment of advanced and unknown gastrointestinal tumors. In minor trauma to detect the disease, pathological diagnosis and appropriate surgical treatment, can reduce the surgical risk of advanced cancer, complications and mortality, shorten the length of stay.
