目的分析组织多普勒显像(TDI)对心肌缺血的价值。方法使用超声心电图方法,记录心肌缺血舒张期二尖瓣血流频谱及左心室基底部、中部和心尖部3个水平面15节段TDI。结果心肌缺血组与正常对照组间相比,左心室射血分值和舒张早期峰值血流速度,差异无统计学意义(P>0.05)。与正常对照组相比,心肌缺血组舒张早期减速度时间和等容舒张时间延长,舒张晚期血流速度加快。组织多普勒显像可见,正常对照组中,左心室心尖部舒张早期节段的心肌舒张速度低于左心室基底部与中部。结论组织多普勒显像技术能够精确反映左心室功能变化,为临床评估心肌缺血提供一个无创性的检查手段。
Objective To analyze the value of tissue Doppler imaging (TDI) in myocardial ischemia. Methods The echocardiography was used to record the mitral inflow and diastolic mitral flow and the TDI of 15 segments in basal, middle and apex of the left ventricle. Results There was no significant difference in left ventricular ejection fraction and early diastolic peak velocity between myocardial ischemia group and normal control group (P> 0.05). Compared with the normal control group, the early diastolic deceleration time and the isovolumic relaxation time in the myocardial ischemia group were prolonged, and the diastolic blood flow velocity was accelerated. Tissue Doppler imaging shows that in the normal control group, the diastolic velocity of the early diastolic segment of the left ventricular apex was lower than that of the basal and middle ventricles of the left ventricle. Conclusion Tissue Doppler imaging can accurately reflect the changes of left ventricular function and provide a noninvasive means for clinical assessment of myocardial ischemia.
