【目的】观察在腹腔镜直肠癌根治术中维持使用地佐辛的镇痛效果。【方法】以2014年6月至2015年10月在本院接受腹腔镜下直肠癌根治术的患者为观察对象。根据其术中维持使用麻醉药物的不同分为瑞芬太尼组和地佐辛组,每组各40例。比较两组患者平均动脉压(MAP)和心率(HR)的变化。观察两组患者术后不同时间点 VAS和 Ramsay评分的差异及患者苏醒时间和拔除气管插管时间及手术前后肝肾功能指标的变化。【结果】两组患者麻醉前 MAP和 H R 相比较无明显差异,手术开始和结束时,两组患者上述指标均较麻醉前降低,且瑞芬太尼组下降更明显(P <0.05);地佐辛组术后4 h、12 h和24 h时的 VAS得分低于瑞芬太尼组(P <0.05),而两组患者的 Ramsay评分无明显差异;地佐辛组患者睁眼时间和气管插管拔管时间均较瑞芬太尼组长,且两组相比较差异具有显著性(P <0.05);两组患者手术前后的肝肾功能指标均无明显差异,且在正常范围内。【结论】腹腔镜直肠癌根治术中维持使用地佐辛有较好的镇痛效果,且对肝肾功能无明显损伤。“,”Obj ective]To observe the analgesic effects of intraoperative dezocine in the operation of laparoscopic radi-cal rectal cancer.[Methods]Eighty patients in our hospital from June 2014 to October 2015 undergoing laparoscopic radi-cal resection of rectal cancer were chosen.According to the respective intraoperative uses of narcotic drugs,the 80 patients were divided into the remifentanil group and dezocine group with 40 patients in each group.The mean arterial pressure (MAP)and heart rate (HR)changes of the two groups were compared,and the the VAS and Ramsay scores at different time points were compared as well.The recovery time,extubation time,and biochemical indexes of liver and kidney func-tion of the two groups were compared.[Results]The HR and MAP of the two groups had no significant difference before anesthesia.However,the indexes of the two groups were lower after anesthesia,and the remifentanil group decreased more significantly than the dezocine group (P <0.05).VAS scores of the dezocine group after 4h,12h,and 24h were lower than remifentanil group (P <0.05),but Ramsay scores of the two groups had no significant difference.The anes-thetic recovery time and extubation time in the dezocine group were higher than in the remifentanil group,the difference was statistically significant(P <0.05).The liver and kidney function indexes,however,had no significantly different be-tween the two groups both before and after operation-all in the normal range.[Conclusion]The use of dezocine in laparo-scopic colorectal cancer operations has good analgesic effects,and has no significant damage to the liver and kidney func-tion.
