目的探讨肿瘤标志物细胞角蛋白19片段(CYFRA21-1)、神经特异性烯醇化酶(NSE)、癌胚抗原(CEA)在肺癌患者血清中的水平,及其在肺癌鉴别诊断中的价值。方法使用罗氏2010电化学免疫分析仪对100例肺癌患者者、69例良性肺疾病(BLD)者及健康体检者3项肿瘤标志物含量进行比较。结果 CYFRA21-1、NSE、CEA3项肿瘤标志物肺癌组水平显著高于BLD患者组及健康体检组(P均<0.01),其水平与病理类型有关。CEA在肺腺癌表达水平最高,CYFRA21-1在肺鳞癌表达水平最高,NSE在肺小细胞癌表达水平最高。联合检测血清CYFRA21-1、NSE及CEA对肺癌的敏感性及准确性显著高于单项检测(P均<0.05)。结论 3项肿瘤标志物对肺癌的检测都有一定意义,不同病理类型各有特点,联合检测更有利于肺癌的鉴别诊断。
Objective To investigate the levels of cytokeratin 19 (CYFRA21-1), neuroendocrine enolase (NSE) and carcinoembryonic antigen (CEA) in the serum of patients with lung cancer and their value in the differential diagnosis of lung cancer. Methods The levels of three tumor markers in 100 patients with lung cancer, 69 patients with benign lung disease (BLD) and healthy people were compared by Roche 2010 electrochemical immunoassay. Results The levels of CYFRA21-1, NSE and CEA in lung cancer group were significantly higher than those in BLD group and healthy control group (all P <0.01), and their levels correlated with pathological types. CEA has the highest expression in lung adenocarcinoma, CYFRA21-1 has the highest expression in lung squamous cell carcinoma and NSE has the highest expression in small cell lung cancer. The sensitivity and accuracy of the combined detection of serum CYFRA21-1, NSE and CEA in lung cancer were significantly higher than those of single detection (all P <0.05). Conclusion The three tumor markers have certain significance for the detection of lung cancer. Different pathological types have their own characteristics. Combined detection is more conducive to the differential diagnosis of lung cancer.
