目的应用常规超声心动图及斑点追踪超声心动图评价阵发性房颤(PAF)患者左心房的大小与功能。方法纳入79例研究对象,阵发性房颤44例,正常对照组35例。常规超声心动图测量左心房最大容积(LAVmax)、最小容积(LAVmin)、左心房收缩前容积(LAVpreA),并计算左心房膨胀指数(LAEI)、左心房被动排空分数(LAPEF)、左心房主动排空分数(LAAEF)。应用2D-STE测量左心房整体长轴应变(GLS)。结果与正常对照组比较,PAF组LAD1、LAD2、LAD2明显增大(P<0.05);LAVImax、LAVImin、LAVIpreA均较正常对照组显著增大(P<0.05)。PAF组LAPEF、LAAEF及LAEI均减低,GLS较正常对照组亦显著减低(P<0.05)。PAF组LVEDVI、LVESVI、肺静脉Ar峰均较正常对照组增大(P<0.05)。结论阵发性房颤患者左心房功能不同程度受损,常规超声心动图测量方法及二维斑点追踪超声心动图相结合可以较好地评估左心房时相功能,有一定的临床意义。
Objective To evaluate the size and function of left atrium in patients with paroxysmal atrial fibrillation (PAF) by conventional echocardiography and speckle tracking echocardiography. Methods A total of 79 subjects were enrolled in this study, including 44 cases of paroxysmal atrial fibrillation and 35 cases of normal control group. Conventional echocardiography was used to measure the left atrium volume (LAVmax), minimum volume (LAVmin), and left atrial appendage volume (LAVpreA), and the left atrial expansion index (LAEI), left atrial passive emptying fraction Active Vacancy Score (LAAEF). Total left atrial long axis strain (GLS) was measured using 2D-STE. Results Compared with the normal control group, LAD1, LAD2 and LAD2 in PAF group were significantly increased (P <0.05); LAVImax, LAVImin and LAVIpreA in control group were significantly higher than those in normal control group (P <0.05). The levels of LAPEF, LAAEF and LAEI in the PAF group decreased, while the GLS in the PAF group was significantly lower than that in the normal control group (P <0.05). LVEDVI, LVESVI, and pulmonary venous Ar peak in PAF group were significantly higher than those in normal control group (P <0.05). Conclusions The left atrial function in patients with paroxysmal atrial fibrillation is impaired to some extent. The combination of conventional echocardiography and two - dimensional speckle tracking echocardiography can evaluate left atrial phase function well and has certain clinical significance.
