目的观察不同类型脐带异常新生儿脐血中神经元特异性烯醇化酶(Neuron Specific Enolase,NSE)水平变化,总结新生儿脑损伤诊治的相关因素。方法选择2013年5月至2014年12月在咸阳市中心医院产科分娩的妊娠期脐带异常新生儿80例为观察组,同期正常出生的新生儿50例为对照组,采用ELISA法检测新生儿脐血NSE水平并进行分析比较。结果观察组脐血NSE含量较对照组明显升高(P<0.05);脐带绕颈组中NSE值与绕颈周数呈正比(P<0.05);脐带打结组中真打结组NSE值高于假打结组(P<0.05);脐带扭转组中NSE值随扭转周数增多而增高(P<0.05)。结论脐带异常可引起脐血中NSE值升高,而脐血NSE水平升高与脑缺氧缺血损伤密切相关,脐血NSE可能做为预测新生儿宫内脑损伤的指标。
Objective To observe the levels of neuron specific enolase (NSE) in umbilical cord blood of different types of umbilical cord abnormalities and to summarize the related factors of diagnosis and treatment of neonatal brain injury. Methods 80 neonates with umbilical cord abnormalities during pregnancy in the Central Hospital of Xianyang City from May 2013 to December 2014 were selected as the observation group. Fifty newborn infants born at the same period were selected as the control group. The navel Blood NSE levels were analyzed and compared. Results The NSE level in umbilical cord blood of the observation group was significantly higher than that of the control group (P <0.05). The value of NSE in the umbilical cord around the neck group was positively correlated with the number of neck circumference (P <0.05) (P <0.05). NSE in umbilical cord torsion group increased with the increase of twisting weeks (P <0.05). Conclusion The umbilical cord abnormality can cause the increase of NSE in umbilical cord blood. The elevated level of NSE in umbilical cord blood is closely related to cerebral hypoxic-ischemic injury. NSE in umbilical cord blood may be used as an index to predict intrauterine brain injury in neonates.
