目的探讨表皮生长因子受体(EGFR)蛋白在宫颈鳞状上皮内瘤变和鳞状细胞癌中的表达及基因扩增,并评估其在宫颈癌发生、发展和预后中的价值。方法分别应用免疫组化EnVision法和荧光原位杂交(FISH)技术检测75例宫颈各种病变组织中的EGFR蛋白表达、基因扩增及基因拷贝数的变化,其中慢性宫颈炎10例,低级别鳞状上皮内瘤变(CIN)16例,高级别CIN 25例,宫颈癌24例。结果 EGFR在高级别CIN和宫颈癌中的阳性表达率分别是76%和79.17%,而在慢性宫颈炎和低级别CIN中的阳性表达率分别是10%和43.75%。高级别CIN和鳞癌中的基因扩增阳性率为25%,而慢性宫颈炎和低级别CIN中的阳性率为3.33%。42例EGFR阴性或低等阳性病例中FISH检测出26例(61.9%)二倍体和11例(26.2%)三倍体,而20例EGFR中等或强阳性病例中却有13例(65%)为多倍体或单纯EGFR基因扩增;所有EGFR基因扩增病例的蛋白表达均为中~强(+)。结论 EGFR可能参与了宫颈癌的发生,并且是宫颈癌发生的早期事件。EGFR蛋白过表达可能由EGFR基因扩增或基因拷贝数增加所致。
Objective To investigate the expression and gene amplification of epidermal growth factor receptor (EGFR) in cervical squamous intraepithelial neoplasia and squamous cell carcinoma and to evaluate its value in the occurrence, development and prognosis of cervical cancer. Methods Immunohistochemical EnVision method and fluorescence in situ hybridization (FISH) were used to detect the expression of EGFR protein, gene amplification and gene copy number in 75 cases of cervical lesions, including 10 cases of chronic cervicitis, low-grade 16 cases of squamous intraepithelial neoplasia (CIN), 25 cases of high grade CIN and 24 cases of cervical cancer. Results The positive rates of EGFR in high grade CIN and cervical cancer were 76% and 79.17% respectively, while the positive rates in chronic cervicitis and low grade CIN were 10% and 43.75% respectively. The positive rate of gene amplification in high grade CIN and squamous cell carcinoma was 25%, while the positive rate in chronic cervicitis and low grade CIN was 3.33%. Twenty-two (61.9%) diploid and 11 (26.2%) triploids were detected by FISH in 42 cases of EGFR-negative or low-positive cases compared with 13 cases of moderate or strong EGFR in 20 cases (65% ) Was polyploidy or simple EGFR gene amplification; all cases of EGFR gene amplification protein expression were moderate to strong (+). Conclusion EGFR may be involved in the occurrence of cervical cancer and is an early event of cervical cancer. EGFR protein overexpression may be caused by EGFR gene amplification or gene copy number increase.
