目的观察不同剂量的重组人促红细胞生成素(rh-EPO)治疗早产儿贫血的临床效果。方法选取2014年3月-2016年3月在该院接受治疗的早产贫血患儿92例为研究对象,并根据随机数字表法将其分为对照组(46例)和观察组(46例)。其中对照组给予低剂量rh-EPO,观察组给予高剂量rh-EPO。观察两组的治疗效果,比较两组患儿治疗前后血常规(RBC、Hb、Hct)和铁蛋白(SF)水平,患儿输血率及家属满意度的差异。结果观察组治疗有效率高于对照组(χ~2=3.866,P<0.05);治疗前两组患儿的RBC、Hb、Hct和SF水平比较差异均无统计学意义(均P>0.05),而治疗后1个月、2个月和3个月时的再次检测结果表明,观察组患儿在治疗后上述时间段的RBC、Hb、Hct和SF水平均高于对照组,差异具有统计学意义(P<0.05);治疗过程中,观察组输血率明显低于对照组(χ~2=4.929,P<0.05),患儿家属满意度明显高于对照组(χ~2=4.039,P<0.05)。结论高剂量rh-EPO对早产儿贫血有较好的治疗效果,可更明显改善患儿的相关实验室指标,降低输血率,提高患儿家属满意度。
Objective To observe the clinical effects of different doses of recombinant human erythropoietin (rh-EPO) on anemia in premature infants. Methods Ninety-two children with preterm anemia who were treated in our hospital from March 2014 to March 2016 were selected and divided into control group (46 cases) and observation group (46 cases) according to the random number table method . The control group was given low-dose rh-EPO, the observation group was given high-dose rh-EPO. The therapeutic effects of the two groups were observed. The blood routine (RBC, Hb, Hct) and ferritin (SF) levels were compared between the two groups before and after treatment. The blood transfusion rate and the satisfaction of their families were compared. Results The effective rate of observation group was higher than that of control group (χ ~ 2 = 3.866, P <0.05). There was no significant difference in RBC, Hb, Hct and SF levels between two groups before treatment (all P> 0.05) , While the retest results at 1 month, 2 months and 3 months after treatment showed that the RBC, Hb, Hct, and SF levels in the observation group were higher than those in the control group after the treatment, the difference was statistically significant (Χ ~ 2 = 4.929, P <0.05). The satisfaction of family members in the treatment group was significantly higher than that in the control group (χ ~ 2 = 4.039, P <0.05). Conclusion High-dose rh-EPO has a good therapeutic effect on anemia in premature infants, and can significantly improve the relevant laboratory indicators in children, reduce blood transfusion and improve the satisfaction of family members.
