目的:探讨肝癌介入治疗中旋转数字减影血管造影(Digital Subtraction Angioraphy,DSA)三维重建技术的应用价值。方法:对我院收治的52例肝癌患者分别行常规DSA和旋转DSA检查,然后将所得旋转DSA图像应用三维重建技术使用多种后处理软件,显示患者的血管和病灶,并与常规DSA图像对比。结果:52例患者中,旋转DSA配合三维重建技术均能够较清晰地显示肿瘤和供血血管之间的关系,并且能清晰显示供血动脉的角度;而常规DSA检查结果中,有12例患者无法清晰显示肿瘤供血动脉,其中2例患者在主病灶周围还存在较小子病灶,但是常规DSA检查无法清晰显示子病灶的供血动脉,两种方法肿瘤和供血血管显示总有效率差异具有统计学意义(P=0.001)。结论:在肝癌介入治疗中,采用旋转DSA三维重建技术具有明显优势,可以清晰显示肿瘤和供血动脉的关系以及供血动脉的角度,是对常规DSA检查的重要补充。
Objective: To investigate the value of three-dimensional reconstruction of digital subtraction angiography (DSA) in the interventional treatment of liver cancer. Methods: We performed routine DSA and rotational DSA examinations on 52 patients with HCC who were treated in our hospital. Then, the obtained rotational DSA images were reconstructed by three-dimensional reconstruction using various post-processing softwares to display the blood vessels and lesions of the patients and compared with the conventional DSA images . Results: Rotating DSA combined with 3D reconstruction technique can clearly show the relationship between the tumor and the blood supply vessel and clearly show the angle of the feeding artery in 52 patients. While in the conventional DSA test, 12 patients can not be clearly identified The tumor-supplying artery was shown in 2 patients. There were also small sub-lesions around the main lesion in 2 patients. However, the conventional DSA could not clearly show the sub-lesion’s feeding artery. The difference between the two methods showed that the total effective rate of tumor and blood supply was statistically significant P = 0.001). Conclusion: The 3D reconstruction technique of rotational DSA has obvious advantages in the interventional treatment of liver cancer, which can clearly show the relationship between the tumor and the feeding artery and the angle of the feeding artery, which is an important supplement to routine DSA examination.
